大黄明矾汤肛内滴注防治痔病术后出血的临床研究

【摘要】：目的:观察大黄明矾汤肛内滴注防治痔病(湿热下注型)术后出血患者的临床疗效。方法:选取2015年12月-2016年12月昆明市中医医院肛肠科72例混合痔术后患者,将其随机分为治疗组和对照组,扣除脱落病例10例,最终有效病例62例,其中治疗组为32例,对照组30例,均予以混合痔外剥内扎术治疗,术后第1天开始用药,治疗组使用大黄明矾汤肛内滴注,每次100ml,每日早、晚各一次,同时口服云南红药胶囊,每次两粒(规格0.25),每天三次,疗程七天;对照组口服云南红药胶囊,每次两粒(规格0.25),每天三次,疗程七天。术后第3天观察记录患者便时出血情况。结果:1.在观察过程中的72例患者,其中有5例因个人原因或失访退出观察,5例患者未按要求服药不计入统计。最终有效病例数62例(治疗组为32例,对照组30例)。治疗组和对照组病例在性别、年龄、病程、手术前病情、治疗前便时出血积分情况等方面无显著性差异(P0.05),在统计学上具有可比性。2.术后第3天、第7天观察两组排便时的出血情况,术后第3天治疗组排便时出血积分少于对照组便时出血积分,两组间比较差异显著有统计学意(P0.05),术后第7天治疗组便时出血积分少于对照组两组比较差异显著有统计学意义(P0.05);术后第7天治疗组显效率和有效率分别为78.12%、96.75%,对照组分别为40%、73.33%,两组显效率、总有效率比较治疗组均高于对照组,差异显著有统计学意义(P=0.0010.05)。3.安全性判定:两组治疗期间均无严重不良反应,呼吸、血压、体温、脉搏等主要生命体征未观察到异常波动。结论:大黄明矾汤具有收敛止血、化瘀止血之功,肛内滴注同时口服云南红药胶囊能明显改善痔病术后排便时出血症状,其中治疗组显效率和有效率分别为78.12%、96.75%,对照组分别为40%、73.33%(P㩳0.05)。临床研究表明大黄明矾汤肛内滴注同时口服云南红药胶囊能减少术后排便时出血症状,提高患者的生活质量。其临床疗效优于单纯口服云南红药胶囊,且不良反应更少,疗效明确,值得临床推广应用。

Clinical studies show that rhubarb decoction in anus drip alum and oral administration of Yunnan Hongyao capsule can reduce postoperative bleeding during defecation. To improve the quality of life of patients. Its clinical curative effect is better than oral administration of Yunnan Hongyao capsule, and less adverse reactions, the effect is clear, it is worthy of clinical application.
