通心络对颈动脉粥样硬化远期疗效的随访研究

【摘要】:目的:通过检测动脉粥样硬化患者颈动脉超声及血脂、血糖、超敏C反应蛋白、纤维蛋白原等临床生化指标,探究中成药通心络对动脉粥样硬化治疗的远期作用。方法:将颈动脉粥样硬化患者108例,随机分为通心络组72例、对照组36例,治疗24个月后将通心络组分为持续通心络组36例、停服通心络组36例,并随访24个月。检测患者基线、服药24个月及48个月后的颈动脉超声指标及血脂、血糖、hs-CRP、Fib等生化指标随时间的变化情况以评估治疗效果。结果:至2015年12月,原108人中106人完成随访,其中1例因出现肝功能异常退出,1例死亡(死于肺部感染),结果表明:1、颈动脉斑块指标:24个月后患者颈动脉B超示通心络组颈动脉斑块内中膜厚度(IMT)及长轴面积(LS)优于对照组(P0.05);斑块短轴面积(SS)两组间差别无统计学意义(P0.05)。随访48个月后,持续通心络组与停服通心络组间颈动脉内中膜厚度(IMT)差异无统计学意义(p0.05),但均优于对照组(P0.05);持续通心络组斑块短轴面积(SS)优于对照组(P0.05);持续服用通心络组斑块长轴面积(LS)优于停服通心络组及对照组(P0.05),余组间差别无统计学意义。2、生化指标:48个月后持续通心络组低密度脂蛋白胆固醇、超敏C反应蛋白水平小于对照组(P0.05)。持续通心络组纤维蛋白原低于对照组(P0.05)及停服通心络组(P0.01),余组间差别无统计学意义。总胆固醇、高密度脂蛋白胆固醇、甘油三酯、谷丙转氨酶、肌酐等指标各组间无明显统计学差异。结论:1.通心络有稳定动脉粥样硬化的作用,可能通过改善低密度脂蛋白胆固醇、超敏C反应蛋白、纤维蛋白原等途径延缓动脉粥样硬化的进程。2.服用通心络一段时间后停服仍能起到稳定斑块的作用,其内中膜厚度仍维持在较低水平。但持续服用通心络有优于服用一段时间后停服通心络的趋势,故应持续服用通心络,以达到最佳的治疗效果。
