祛风安神止痛针刺法治疗紧张型头痛的临床观察

【摘要】：目的:探究祛风安神止痛针刺法治疗紧张型头痛的临床疗效。方法:将符合纳入标准的60例紧张型头痛患者依随机数字表法分为治疗组与对照组。治疗组30例,给予祛风安神止痛针刺法,选取百会、左右神聪、印堂、水沟、膻中、双侧的太阳、头维、风池、内关、神门穴。对照组30例患者进行常规针刺法,针刺百会,双侧的太阳、头维、风池、颈夹脊、足三里、太冲等穴。两组均每日治疗1次,每次50 min,7日1个疗程,连续4个疗程。分别于治疗前、治疗2周与治疗结束后评定两组患者的视觉模拟评分(VAS评分)并采集患者治疗前、后头痛强度积分和汉密顿抑郁(HAMD)评分,观察各项指标的变化情况,并计算总有效率。结果:1.VAS评分:两组患者治疗2、4周时VAS评分与该组治疗前评分的差值分别做组间比较,有非常显著性差异(P0.01)。2.头痛强度积分:两组患者治疗前、后头痛强度积分组内比较,均差异非常显著(P0.01);两组患者治疗4周后头痛强度积分组间比较亦有显著性差异(p0.05)。3.HAMD评分:治疗组治疗前、后HAMD评分组内比较,有非常显著性差异(P0.01);对照组治疗前、后组内比较有显著性差异(p0.05);两组治疗后组间比较有显著性差异(p0.05)。4.两组临床整体疗效的比较:治疗组总有效率为90.00%;对照组的总有效率为66.67%,二者之间有显著性差异(P0.05)。结论:1.祛风安神止痛针刺法可减轻紧张型头痛患者的疼痛症状,且相较于常规针刺法起效快、止痛疗效更为显著。2.祛风安神止痛针刺法可很好改善紧张型头痛患者的抑郁状态,而抑郁状态的好转也有助于头痛症状的减轻。
[Abstract]:Objective: to investigate the clinical curative effect of the acupuncture method of dispelling wind and tranquilizing the spirit and relieving pain on tension-type headache. Methods: according to the random digital table, 60 patients with tension-type headache were divided into the treatment group and the control group, 30 cases in the treatment group, 30 cases in the treatment group, and 30 cases in the control group. Give the acupuncture method of dispelling wind and calming the spirit and relieving pain, select Baihui, left and right Shencong, Yin Tang, ditch, Tanzhong, bilateral sun, head dimension, wind pond, Neiguan and Shenmen acupoints. The two groups were treated once a day for 50 mins, 7th for a course of 4 consecutive courses of treatment, respectively, before treatment, The visual analogue score and VAS score of the two groups were evaluated after 2 weeks of treatment and the end of treatment. The scores of headache intensity and Hamd before and after treatment were collected to observe the changes of the indexes. Results 1. VAS score: the difference of VAS score between the two groups at 24 weeks after treatment was significantly different from that before treatment in two groups (P < 0.01). Headache intensity score: before treatment, there was a significant difference between the two groups (P < 0.05), and there was a significant difference between the two groups (P < 0.05), and there was a significant difference between the two groups (P < 0.05). After 4 weeks of treatment, there were significant differences in the scores of headache intensity between the two groups. 3. Hamd scores: HAMD scores before and after treatment in the treatment group were significantly higher than those in the control group, and there were significant differences between the two groups before and after treatment. There was a very significant difference (P 0.01); before treatment, in the control group, There was a significant difference between the two groups after treatment. Conclusion (1) the acupuncture method of dispelling wind and soothing the spirit and relieving pain can relieve the pain symptoms of the patients with tension headache. Compared with the conventional acupuncture method, the analgesic effect is more obvious. 2. The acupuncture method of dispelling wind and soothing the mind and relieving pain can improve the depression state of the patients with tension headache, and the improvement of the depression state can also help to alleviate the headache symptoms.
