补肾调周法治疗肾阴虚型卵巢储备功能下降的临床研究

【摘要】：研究目的:1.讨论分析夏氏补肾调周法治疗肾阴虚型卵巢储备功能下降的临床疗效及治疗药物的安全性。2.改善患者临床症状,恢复正常月经,为患者维持正常的生活质量奠定基础。3.提高正常受孕及辅助生殖成功率。4.为临床中进一步推广、运用提供依据。研究方法:本研究采用自身前后对照法,收集符合纳入病例标准的卵巢储备功能下降(肾阴虚型)患者30例,运用补肾调周法治疗6个月。治疗前与治疗后相对比,研究分析患者的月信周期长短、经量多少及中医临床症状是否有变化、变化的程度等,同时观察血清黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、卵泡刺激素/黄体生成素(FSH/LH)比值的变化、B超变化及患者妊娠情况,来分析补肾调周法治疗卵巢储备功能下降的机理及疗效。结果:1.本研究所纳入30例患者,经夏氏补肾调周法治疗后,痊愈3人,比例为10.0%,显效7人,比例为23.3%,有效17人,比例为56.7%,总有效率达90%;治疗后中医证候疗效总有效率为90%,与治疗前相比,统计有明显差异(P0.05)。2.相比于治疗前,治疗6月后血清E2水平升高,FSH下降,FSH/LH比值降低,治疗前后统计差别显著(P0.05)。治疗前后LH数值无明显统计学差异(P0.05)。3.治疗2个疗程后中医症状总积分为12.27±9.21,与治疗前中医症状总积分28.13±4.98对比,显然下降(P0.05),且治疗后患者经期、经量、腰膝酸软、心悸耳鸣、烦躁失眠、易怒多梦及潮热盗汗等症均较前明显改善,差异有统计学意义(P0.05)。结论夏氏补肾调周法治疗肾阴虚型卵巢储备功能低下,成效显著。运用夏氏补肾调周法治疗DOR,在促进患者临床症状向愈、生活质量向好、性激素水平趋于平衡及月经周期趋于正常等方面,效果突出;夏氏补肾调周法可通过调节心-肾-子宫生殖轴,来调节机体内环境,从而达到改善卵巢功能的作用。
[Abstract]:Objective 1. To discuss and analyze the clinical effect of Xia's method of tonifying the kidney and regulating the week in the treatment of the decline of ovarian reserve function in the type of kidney yin deficiency and the safety of the medicine. 2. To improve the clinical symptoms of the patients and restore their normal menstruation. To lay a foundation for patients to maintain normal quality of life .3. to improve the success rate of normal conception and assisted reproduction. 4. To provide the basis for further promotion and application in clinical practice. Thirty patients with decreased ovarian reserve function (kidney yin deficiency type) were collected and treated for 6 months with tonifying kidney and regulating week. At the same time, the changes of serum luteinizing hormone (LHN), follicle stimulating hormone (FSH), estradiol estradiol (E2N), follicle stimulating hormone / luteinizing hormone (FSH / LH) ratio, B-mode ultrasound and pregnancy were observed. To analyze the mechanism and curative effect of tonifying kidney and regulating week in treating the decline of ovarian reserve function. Results 1. 30 patients were included in this study. 3 patients were cured after treatment with Xia's method of tonifying kidney and regulating week, the proportion was 10.0, the effect was 7, the proportion was 23.3g, and the effective was 17. The ratio was 56.7 and the total effective rate was 90. After treatment, the total effective rate of TCM syndromes was 90, compared with that before treatment, there was a significant difference between before treatment and before treatment. Compared with before treatment, the level of serum E2 increased after treatment on June, and the ratio of FSH to FSH decreased. There was no significant difference in LH value before and after treatment. The total score of TCM symptoms after two courses of treatment was 12.27 卤9.21, which was significantly lower than that before treatment (28.13 卤4.98). The waist and knee ache, palpitation and tinnitus, restlessness and insomnia, irritability and dreams, hot flashes and night sweats were all significantly improved, and the difference was statistically significant (P 0.05). Conclusion Xia's method of tonifying the kidney and regulating the week in the treatment of ovarian reserve function of kidney yin deficiency type is lower than that before. The effect is remarkable. The treatment of Dor with Xia's method of tonifying the kidney and regulating the week is effective in promoting the recovery of clinical symptoms, the quality of life, the balance of sex hormone level and the normal menstrual cycle. Xia's method of tonifying the kidney and regulating the week can regulate the internal environment of the body by regulating the reproductive axis of heart kidney and uterus so as to improve the function of ovary.
