健脾理气化浊方治疗酒精性脂肪肝的疗效观察

【摘要】：目的:观察健脾理气化浊方治疗酒精性脂肪肝的临床疗效,期望能为中医治疗酒精性脂肪肝提供新的思路。方法:按照严格的入选标准及排除标准,选取80例AFL患者,随机分为中药组和西药组,每组各40例,两组患者性别、平均年龄、饮酒年限、原发病情况等基本一致,具有可比性(P0.05)。中药组给予中药健脾理气化浊方治疗,日1剂,早晚水煎约300ml服用,西药组给予口服多烯磷脂酰胆碱胶囊治疗,每日3次口服,每次2粒,疗程均为12周。比较治疗前后肝脏脂肪变性程度,肝功能指标ALT、AST、GGT,血脂TC、TG及中医症候积分的变化,并作出评价。结果:1.两组治疗前后肝脏脂肪变性程度:中药组有明显改善(P0.01),西药组有改善(P0.05),但中药组优于西药组(P0.05)。2.两组治疗前后肝功能比较:中药组组内比较ALT、AST、GGT均明显降低(P0.01),西药组组内比较ALT、AST、GGT差异有统计学意义(P0.05),且中药组优于西药组(P0.05)。3.两组治疗前后血脂比较:两组治疗前后TC、TG组间对比具有统计学意义(P0.05),且中药组优于西药组(P0.05)。4.两组治疗前后中医症候积分比较:中医症候积分属轻度者,两组积分无明显差异(P0.05),但中重度者中医证候积分有显著差异(P0.05)。5.两组总体疗效比较:中药组总有效率为97.5%,西药组总有效率为75%,差异有统计学意义(P0.05),说明中药组疗效明显优于西药组。结论:健脾理气化浊方对于酒精性脂肪肝患者有显著的疗效,并能在很大程度上改善肝脏细胞的脂肪化,治疗总体有效率明显高于单独服用多烯磷脂酰胆碱胶囊。
[Abstract]:Objective: to observe the clinical effect of Jianpi Liqi Zhuo recipe in the treatment of alcoholic fatty liver, hoping to provide a new idea for the treatment of alcoholic fatty liver in TCM. Methods: according to the strict selection criteria and exclusion criteria, 80 patients with AFL were selected. They were randomly divided into traditional Chinese medicine group and western medicine group with 40 cases in each group. The two groups had the same sex, average age, drinking years, primary disease and so on. The traditional Chinese medicine group was treated with traditional Chinese medicine Jianpi liqi-zhuo prescription, 1 dose per day. Morning and evening decoction about 300ml, western medicine group was given oral polyene phosphatidylcholine capsule, oral three times a day, each time 2 capsules, the course of treatment was 12 weeks. The degree of hepatic steatosis was compared before and after treatment. The changes of liver function index (alt) ASTT GGTT, TG of blood lipid and TCM symptom integral. Results: before and after treatment, the degree of hepatic steatosis in Chinese medicine group and western medicine group was significantly improved, but it was better than that in western medicine group before and after treatment. The liver function of the two groups was compared before and after treatment: the liver function of traditional Chinese medicine group was compared with that of alt ASTGGT group. The difference of GGT between western medicine group and western medicine group was statistically significant (P 0.05), and that of traditional Chinese medicine group was better than that of western medicine group (P 0.05). 3. Comparison of blood lipids between two groups before and after treatment: the comparison between two groups before and after treatment had statistical significance (P 0.05), and the traditional Chinese medicine group was better than western medicine group (P 0.05). Comparison of TCM symptom score before and after treatment between the two groups: the score of TCM symptom was mild, There was no significant difference in the scores between the two groups, but there was a significant difference in the score of TCM syndromes between the two groups. The total effective rate of the two groups was 97.5g in the traditional Chinese medicine group and 75cm in the western medicine group, indicating that the curative effect of the traditional Chinese medicine group was higher than that of the traditional Chinese medicine group (P 0.05). Conclusion: Jianpi Liqi zhuo prescription has significant curative effect on alcoholic fatty liver. The total effective rate of treatment was significantly higher than that of polyenylphosphatidylcholine capsule alone.
