基于B超下不同分型观察推拿治疗先天性肌性斜颈的临床疗效

【摘要】：目的:观察推拿对B超下不同分型先天性肌性斜颈患儿Cheng和Lee评分量表、B超评分的影响及临床疗效,探索可能存在的机制,为推拿治疗B超下不同分型先天性肌性斜颈提供客观依据。方法:本研究在B超引导下筛选出符合标准的先天性肌性斜颈患儿70例,治疗A组(肿块型)35例,治疗B组(均匀增厚型)35例;两组均予以推拿手法治疗,4个周期后观察疗效,并比较两组Cheng和Lee评分量表中各项评分每个周期的变化,治疗前后两组B超评分的变化,观察综合疗效。结果:1治疗A组痊愈11例(32.35%),好转21例(61.76%),无效2例(5.88%),总有效率94.12%;治疗B组痊愈23例(67.65%),好转10例(29.41%),无效1例(2.94%)总有效率97.06%;两组总有效率之间有差异(P㩳0.05)。2自第二周期起,两组Cheng和Lee评分表各指标组间及组内两两比较有差异(P㩳0.05);两组B超评分组间及组内比较有差异(P㩳0.05)。结论:推拿对先天性肌性斜颈两种证型均有效,总有效率治疗B组优于治疗A组,其疗效确切、安全可行、副作用甚小、绿色环保,值得临床广泛应用、推广。
[Abstract]:Objective: to observe the effect of massage on Cheng and Lee scores and clinical efficacy in children with congenital torticollis under different types of B-ultrasound, and to explore the possible mechanism. Methods: 70 children with congenital torticollis were selected under the guidance of B-mode ultrasound and 35 patients in group A were treated with massage. Group B (35 cases of homogeneous thickening type) were treated with massage manipulation, and the curative effect was observed after 4 cycles. The changes of each cycle in Cheng and Lee scale were compared, and the changes of B-ultrasound score before and after treatment were compared. Since the second cycle, there have been differences in Cheng and Lee scores between and within the two groups. There was a significant difference between the two groups in B-ultrasound score and in group A (P < 0.05). Conclusion: massage is effective for congenital muscular torticollis, and the total effective rate in group B is better than that in group A. it is effective, safe and feasible, with little side effect, green environmental protection, and is worthy of extensive clinical application and popularization.
