朝医方“万金文武汤”加减治疗2型糖尿病的临床研究

【摘要】：目的：通过朝医方《万金文武汤》加减对太阴人2型糖尿病(气阴两虚型)的临床疗效分析,评价朝医药在治疗2型糖尿病的有效性和临床价值,并为朝医治疗该病提供科学依据。方法：将2014年1月至2016年1月期间,延边朝医医院和延边中医医院门诊及住院部收治的太阴人2型糖尿病患者60例,随机分为治疗组和对照组各30例,两组均采用基础治疗,糖尿病知识教育、个体化的营养治疗和运动疗法。对照组口服二甲双胍,每次0.5g,每日3次；治疗组在口服二甲双胍基础上加服万金文武汤(海松子12g、葛根20g、黄芩12g、藁本12g、天门冬6g、麦门冬6g、桔梗6g、白芷6g、大黄6g、升麻6g、莱菔子6g五味子6g)治疗。4周为1个疗程,治疗3个疗程后,观察临床症状变化并统计分析比较治疗前后空腹血糖、餐后2小时血糖、糖化血红蛋白、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、中医症状积分等指标的变化情况,根据疗效判定标准,进行疗效评价。结果：总有效率：治疗组优于对照组(P0.05)；临床症状：治疗组改善临床症状的程度明显优于对照组(P0.05)；血糖及糖化血红蛋白：两组均能降低FPG、2hPG、HbAlc,治疗组明显较对照组的疗效更高(P0.05)；血脂：治疗组能够明显调节异常改变的血脂,其改善效果优于对照组(P0.05)。结论：朝医方《万金文武汤》加减联合西药治疗太阴人2型糖尿病疗效确切,在控制血糖,改善临床症状等方面优于单纯西医药治疗。
[Abstract]:Objective: to evaluate the effectiveness and clinical value of Korean medicine in the treatment of type 2 diabetes mellitus (deficiency of qi and yin) by adding and subtracting Wanjin Wenwu decoction. Methods: from January 2014 to January 2016, 60 patients with type 2 diabetes mellitus of Taiyin were treated in Yanbian Korean Medical Hospital and Yanbian traditional Chinese Medicine Hospital. They were randomly divided into treatment group (n = 30) and control group (n = 30). Both groups were treated with basic treatment, knowledge education of diabetes mellitus, individualized nutrition therapy and exercise therapy. The treatment group was treated with Wanjin Wenwu decoction (sea pine seed 12g, Pueraria lobata 20g, Scutellaria baicalensis 12g, ligustilium ligusticum 12g, Aspergillus asparagus 6g, Radix angelicae dahurica 6g, Rhubarb 6g, Radix Cassiae 6g, Raphani seed 6g Schisandra chinensis 6g) on the basis of oral metformin. After 3 courses of treatment, the changes of clinical symptoms were observed and compared with those before and after treatment, including fasting blood glucose, 2 hours postprandial blood glucose, glycosylated hemoglobin, total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol. The change of TCM symptom integral and other indexes, according to the criteria of curative effect, Results: the total effective rate: the treatment group was superior to the control group (P 0.05N); the clinical symptom: the treatment group improved the clinical symptoms obviously than the control group (P 0.05); the blood sugar and glycosylated hemoglobin: both groups could reduce the FPG2hPGN HbAlc, the treatment group was obviously better than the control group. The therapeutic effect was higher than that of the control group (P 0.05). Blood lipid: the treatment group was able to regulate the abnormal blood lipid obviously. Conclusion: the combination of Chinese medicine and western medicine is effective in the treatment of Taiyin type 2 diabetes mellitus. It is superior to western medicine in controlling blood sugar and improving clinical symptoms.
