四黄痔疮膏治疗混合痔（湿热下注型）术后常见并发症的临床研究及对羟脯氨酸的影响

【摘要】：目的:观察四黄痔疮膏治疗湿热下注型混合痔术后常见并发症的便时疼痛、肛缘水肿、便血、渗液、创面愈合率的临床疗效观察及新生肉芽组织中羟脯胺酸含量,旨在缩短混合痔术后恢复时间,减轻患者痛苦,探索四黄痔疮膏促进创伤修复与羟脯胺酸的关系。方法:选择符合纳入标准的60例均行混合痔外剥内扎术的患者按根据随机数字表法分为治疗组、对照组两组,每组各例30例。两组术后均采相同的基础治疗,治疗组予以四黄痔疮膏换药;对照组予马应龙麝香痔疮膏换药,疗程为11天。观察术后治疗前及治疗后便时肛门疼痛、水肿、便时出血、渗液的症状积分下降值及便时肛门疼痛起效时间和消除时间,术后11天时创面愈合率;第6天时治疗组、对照组按照随机数字表法各取10例肉芽组织测定羟脯胺酸的含量。结果:(1)术后第6天治疗组在缓解便时疼痛、改善水肿、减少便时出血及创面渗液方面均优于对照组(P0.05)具有统计学意义,术后11天各症状无差异,无统计学意义。(2)术后便时疼痛起效、消除时间均优于对照组(P0.05)。(3)术后11天治疗组创面面积减少率均高于对照组(P0.05),差异明显,有统计学意义,术后11天治疗组四黄痔疮膏观察总体疗效率分别为96.67%,对照组马应龙麝香痔疮膏观察总体有效率为86.67%,治疗组与对照组比较存在显著性差异(P0.05),治疗组(四黄痔疮膏)效果明显好于对照组(马应龙麝香痔疮膏),术后第6天肉芽组织中羟脯胺酸含量高于对疗组(P0.05),差异有统计学意义,说明治疗组创面愈合时间短、愈合快可能与羟脯胺酸含量增加有关。结论:四黄痔疮膏具有清热利湿,行气消肿,活血止痛之功,外用治疗湿热下注型混合痔术后能明显改善术后便时疼痛、肛缘水肿、便时出血、渗液等症状,减少便时疼痛时间,增加羟脯胺酸含量促进创面肉芽生长,缩短创面愈合时间,缩短术后疗程,减轻患者痛苦。
[Abstract]:Objective: To observe the four yellow hemorrhoids ointment damp type mixed hemorrhoid postoperative common complications of pain, hematochezia, anal edema, exudation, hydroxyproline content of the wound healing rate of clinical curative effect and granulation tissue, to shorten the recovery time of postoperative mixed hemorrhoids, relieve the pain of patients, explore four Huang hemorrhoids ointment to promote wound repair and hydroxyproline. Methods: 60 cases met inclusion criteria were mixed hemorrhoids patients according to randomly divided into treatment group and control group two groups, each group of 30 patients were collected. Results: (1) after sixth days of treatment group improved in relieving pain and edema, and reduce bleeding and wound exudate were better than the control group (P0.05) with statistical significance, 11 days after operation the symptoms had no difference, no statistical significance. (2) after the operation when the pain onset, the elimination time was better than the control group (P0.05). (3) after 11 days of treatment group were higher than that of the wound area reduction rate the control group (P0.05), significant difference was statistically significant, 11 days after treatment group four yellow ointment observation total therapeutic rate was 96.67%, the control group of Musk Hemorrhoids Ointment observe the total effective rate is 86.67%, compared with the control group, there was significant difference (P0.05), treatment group (four yellow hemorrhoids cream) effect Significantly better than the control group (mayingnong cream), hydroxyproline content after sixth days of granulation tissue was higher than that of treatment group (P0.05), the difference was statistically significant, indicating that the treatment group wound healing time is short, fast healing may be associated with increased hydroxyproline content. Conclusion: four yellow hemorrhoids ointment has heat and dampness, Qi blood swelling, alleviating pain, topical treatment of damp heat type mixed hemorrhoid surgery can significantly improve the postoperative and pain, anal edema, hemorrhage and exudation, and other symptoms, reduce pain, increase hydroxyproline content and promote wound granulation tissue growth, shorten the wound healing time, shorten the postoperative treatment, to alleviate the suffering of patients.
