自然态鼻咽癌中医舌象及经络特征初步研究

【摘要】：目的：探寻自然态鼻咽癌患者中医舌象以及经络特征。方法：舌象研究：病例选用未行治疗自然态鼻咽癌患者40例,男22例,女18例,最小年龄24岁,最大70岁,平均53.7岁。健康体检人80例为对照组,男37例,女43例,最小年龄22岁,最大年龄65岁,平均51.2岁。舌象的拍照采用相机尼康D70拍摄(最大像素800万),采用自然光线拍照,患者在安静状态,自然伸舌分别在自动白平衡和手动白平衡模式下拍摄舌象。淡红舌为正常舌象,异常舌象包括：淡白舌为虚象,红舌为热象,紫暗舌为瘀象。经络研究：鼻咽癌30例,鼻咽癌为自然态患者,未进行放化疗及手术等治疗,其中男20例,女10例,平均年龄41.0±10.45岁。慢性鼻咽炎20例,其中男8例,女12例,平均年龄40.20±10.60岁,使用第三代DMS中医经络检测仪(型号：SMF-Ⅲ,芜湖圣美孚科技有限公司)对患者的特定五输穴对十二经络进行检测,以经络能量来衡量各经络的虚实状态。柱状图超过H2者定义为实证,柱状图位于H1.和H2之间者定义为偏实证,低于L2者定义为虚证,位于L1和L2之间者偏虚证,实证组包括实和偏实,虚证组包括虚和偏虚。位于H1和Ll之间称为正常。结果：鼻咽癌异常舌象70%(28/40),健康体检者异常舌象31.25%(25/80),鼻咽癌患者更易出现异常舌象,统计学分析有显著性差异(x2=16.102,P0.01),异常舌之淡白舌在鼻咽癌出现的比例为40%(16/40),而在健康体检者出现的比例为3.75%(3/80),统计学分析有显著性差异(x 2=26.07,P0.01)。异常舌之红舌在鼻咽癌出现的比例为20% (8/40),而在健康体检者出现的比例为25% (20/80),统计学分析无显著性差异(x2=0.37,P0.05),异常舌之紫色舌在鼻咽癌出现的比例为10%(4/40),而在健康体检者出现的比例为2.5%(2/80),统计学分析无显著性差异(x2=3.13,P0.05)。鼻咽癌与鼻咽炎经络检查结果显示,鼻咽癌虚证多出现于肝经、三焦经及大肠经,鼻咽癌肝经出现虚证比例为7/60,而鼻咽炎出现肝经虚证为0/40,x2=4.962,P0.05,鼻咽癌三焦经出现虚证比例为44/60,,而鼻咽炎出现三焦经虚证为12/40,x2=18.643,P0.01,鼻咽癌大肠经出现虚证比例为37/60,,而鼻咽炎出现大肠经虚证为14/40,x2=10.433,P0.01。鼻咽癌与鼻咽炎相比较,鼻咽癌实证多出现脾经、胃经及肾经,鼻咽癌脾经出现实证比例为24/60,而鼻咽炎出现脾经实证为6/40,X 2=8.046,P0.01,鼻咽癌胃经出现实证比例为11/60,,而鼻咽炎出现胃经实证为2/40,x2=8.132,P0.01,鼻咽癌肾经出现实证比例为10/60,,而鼻咽炎出现胃经实证为0/40,x2=7.326,P＜0.01。而其它经络的虚证及实证在鼻咽癌及鼻咽炎两组患者无显著性差异。结论：自然态鼻咽癌患者异常病理舌象多以淡白舌多见。自然态鼻咽癌患者经络虚证多出现于肝经、三焦经及大肠经,实证多出现于脾经、胃经及肾经。
[Abstract]:Objective: to explore the characteristics of tongue and meridian in patients with natural nasopharyngeal carcinoma.Methods: tongue study: 40 cases (22 males and 18 females) with natural nasopharyngeal carcinoma without treatment were selected. The youngest age was 24 years old, the maximum was 70 years old (mean 53.7 years).There were 80 healthy persons as control group, 37 males and 43 females, the minimum age was 22 years old, the maximum age was 65 years old, the average age was 51.2 years old.The picture of tongue was taken by Nikon D70 camera (maximum pixel 8 million, natural light was used, the patient was in quiet state, and the tongue was shot in automatic white balance mode and manual white balance mode respectively).Pale red tongue is normal tongue image, abnormal tongue image includes: pale white tongue is virtual image, red tongue is heat image, purple dark tongue is stasis image.Meridian study: there were 30 cases of nasopharyngeal carcinoma (NPC). The patients were not treated with radiotherapy, chemotherapy and surgery, including 20 males and 10 females, with an average age of 41.0 卤10.45 years.There were 20 cases of chronic nasopharyngitis, including 8 males and 12 females, with an average age of 40.20 卤10.60 years.
