【摘要】：目的:探寻吾师继承马智教授“调肝脾,安心神”治疗不寐之经验并将之发展应用于消渴病伴发不寐治疗中的辨证思想和遣方用药特色,为中医治疗此病注入新思路。材料与方法:通过查阅吾师跟随马智教授学习期间之资料,及弟子跟师学习期间吾师所授经验与对临床典型病例的积累记录,系统分析并总结,以归纳吾师特色治疗消渴病伴发不寐。结果:1.吾师对消渴病伴发不寐病因病机认识:消渴病之本以气阴两虚为主,标则以燥热血瘀为重,不寐可发生于消渴病的任意发展阶段,由多种因素影响致心神失养或心神受扰而不寐,其中多以肝脾为起病之源,病位在心。2.吾师辨证论治消渴病伴发不寐:(1)治疗过程中重视辨清虚实,强调补气益阴之基础性;(2)以“调肝脾,安心神”为治疗原则,指导后续遣方用药;(3)根据病程发展及临床表现将此病分为阴虚内热,心神不宁;心肾不交,心神失养;心脾两虚,心神失养;气阴两虚,肝郁化火扰神;痰热内扰,热扰心神5种证型,辨证论治。结论:吾师对消渴病伴发不寐之中医治疗,临床疗效显著,具有学习研究价值。
[Abstract]:Objective: to explore our teacher's experience in treating insomnia by inheriting Professor Ma Zhi's "regulating liver and spleen, soothing the heart and mind" and applying it to the dialectical thinking of syndrome differentiation and the characteristics of prescribing prescriptions in the treatment of dysentery associated with insomnia, so as to inject new ideas for the treatment of this disease by traditional Chinese medicine.Materials and methods: by consulting the data of my teacher during his study with Professor Ma Zhi, the experience of my teacher and the accumulation of typical clinical cases during the study of my disciple and teacher, a systematic analysis and summary were made.In order to sum up the characteristics of my teacher in the treatment of thirst accompanied by insomnia.The result is 1: 1.My teacher's understanding of the etiology and pathogenesis of diabetes associated with insomnia: the root of the disease is deficiency of qi and yin, the standard is dryness, heat and blood stasis, and insomnia can occur at any stage of the development of the disease.Many factors affect the heart and mind loss of nourishment or disturbance of the heart and mind, in which the liver and spleen are the source of the disease, and the disease is located in the heart. 2.In the course of treatment, we pay attention to the differentiation of deficiency and reality, and emphasize the foundation of replenishing qi and nourishing yin. 2) "regulating the liver and spleen, tranquilizing the heart and mind" is the treatment principle.According to the course of disease and clinical manifestations, the disease is divided into Yin deficiency and internal heat, heart and heart restlessness; heart and kidney disconnection, heart and spirit lose nourishment; heart and spleen deficiency, heart and spirit loss of nourishment; qi and yin deficiency, liver depression and fire disturbance; phlegm internal disturbance;Heat disturbs the heart spirit 5 kinds of syndromes, dialectical treatment.Conclusion: the clinical effect of my teacher on the treatment of dysentery accompanied with insomnia is significant and has the value of learning and research.
