手法对外周炎性痛大鼠局部镇痛作用及其机制的实验研究

【摘要】:目的:通过观察中医骨伤科理筋手法对完全弗氏佐剂性炎症痛大鼠疼痛阈值的影响及测量手法局部治疗后腺苷的含量,探讨手法缓解炎症痛的效果及其镇痛作用机制,为临床理筋手法局部镇痛作用提供理论依据。方法:选取健康SD雌性大鼠24只,随机分为空白组(A1)、弗氏佐剂模型组(A2)、手法治疗组(A3),每组均为8只。A1空白组不给予造模处理,亦不给予手法治疗,在实验开始以后,将大鼠用固定器进行固定,固定时间与手法治疗组相同;A2弗氏佐剂模型组于实验开始第一天,在大鼠右后足垫部皮下注射完全弗氏佐剂0.1ml致炎,建立大鼠佐剂性炎症疼痛模型,不给予治疗,其它处理同空白组。A3手法治疗组于实验开始第一天,在大鼠右后足垫部皮下注射弗氏完全佐剂0.1ml致炎,建立大鼠佐剂性炎症疼痛模型,并于造模后第4日对大鼠进行手法治疗,手法选取理筋手法中常用的中指点压法。在安静环境中,将大鼠固定在特制的动物固定器上,头部及身体固定,使动物处于清醒状态下,选取足部涌泉穴给予点压法治疗,时间20min,治疗完毕后测量并记录大鼠热痛阈值,然后处死取材。通过测量大鼠的热敏缩足反射时间观察痛阈值改变,测量脚掌周径以评估大鼠足肿胀程度对模型进行评价。分别于三个时间点(造模前1天,造模后4天治疗前,治疗后)应用HWS-26电热恒温水浴锅测定大鼠热疼痛阈值,并制作脚掌组织病理切片,手法干预后用ELISA试剂盒检测腺苷的含量。结果:1.大鼠一般情况:空白组大鼠皮毛光润,精神状态好,喜动,反应灵敏,饮食正常;弗氏模型组A2、手法治疗组A3造模后,大鼠致炎侧足部出现红肿,24h左右达到高峰,皮毛失去光泽,精神萎靡,少活动,致炎侧足部不敢着地,呈跛行,持续3天后症状逐渐减轻。2.造模前各组大鼠之间脚掌周径比较,差异不明显(P0.05),造模后,模型大鼠脚掌周径明显增加,24h左右达到高峰,与对侧脚掌周径比较,有显著性差异(P0.05),与空白组脚掌周径比较,有显著性差异(P0.05)。3.组织病理学研究显示:模型大鼠肌肉组织可见炎性细胞浸润,肌纤维的萎缩,肌纤维间隙增宽,小血管增生等病理改变等,与正常组比较,有明显差异。4造模前大鼠各组间热痛阈值比较,无显著性差异(P0.05);造模后各组大鼠热痛阈值,弗氏模型组A2、手法治疗组A3与空白组A1比较,明显降低,均有显著性差异(P0.05);手法治疗组A3治疗后热痛阈值明显升高,与治疗前比较有显著性差异(P0.05);各组大鼠治疗后热痛阈值比较,空白组A1与其他各组比较均有显著性差异(P0.05),弗氏模型组A2与手法治疗组A3比较均有显著性差异(P0.05);治疗后各组大鼠腺苷浓度,空白组A1与弗氏模型组A2比较无显著性差异(P0.05),手法治疗组A3大鼠外周组织腺苷含量明显升高,与空白组A1比较有显著性差异(P0.05),与模型组A2比较有显著性差异(P0.05)。结论:1.完全弗氏佐剂作为一种抗原复合物,可以使大鼠足部注射部位局部产生红、肿及热痛敏行为反应,并引起组织病理学的改变,所造模型是一种常用的外周炎症痛模型。2.理筋手法局部镇痛效果显著,能够显著提高炎性疼痛模型大鼠的热疼痛阈值。3.理筋手法治疗能够升高炎症疼痛大鼠局部神经递质腺苷浓度,理筋手法局部镇痛作用可能通过局部腺苷释放而起作用。
[Abstract]:Objective: To observe the content of adenosine by TCM traumatology according to massage effect on pain threshold in rats with complete Freund's adjuvant induced inflammatory pain and measurements after local treatment practices to alleviate inflammation pain effect and its mechanism of analgesic effect, for clinical manipulation for local analgesia and provide a theoretical basis. Methods: healthy female SD 24 rats were randomly divided into control group (A1), Freund's adjuvant model group (A2), massage group (A3), each group had 8 rats in.A1 group were not given blank model, nor give treatment, at the start of the experiment, the rats were treated with fixation fixation. Fixed time and manipulative treatment group; A2 Freund's adjuvant model group at the beginning of the first day, in the rat right hind foot pad subcutaneous injection of complete Freund's adjuvant 0.1ml induced inflammation, establishment of rat adjuvant inflammatory pain model, not treated, other treatment with the control group.A3 Manipulative treatment group at the beginning of the experiment the first day, the foot pad of the subcutaneous injection of complete Freund's adjuvant induced arthritis in 0.1ml rats right after the establishment of rat adjuvant induced inflammatory pain model, and on the fourth day after the rats were subjected to treatment, technique selection pointing pressure method commonly used in manipulation in. In the quiet environment, the rats were fixed in the special animal fixer, head and body fixed, the animal in the awake state, select the foot Yongquan point to pressure therapy, treatment time 20min, after the measurement and recording of rat heat pain threshold, and then sacrificed. Muscle Fiber atrophy, muscle fiber gap widened, vascular proliferation and other pathological changes, compared with the normal group, there was significant difference between.4 rats and heat pain threshold comparison mode, no significant difference (P0.05); after the model rats heat pain threshold, the Freund model group A2 technique the treatment group with the control group, A3 A1, was significantly reduced, there were significant differences (P0.05) technique; treatment group A3 thermal pain threshold was significantly increased after treatment, there was significant difference compared with before treatment (P0.05); compare the thermal pain threshold of rats after the treatment, the control group of A1 and the other groups were significant the difference (P0.05), model group A2 and Freund's manipulative treatment group A3 had significant difference (P0.05); adenosine concentration in rats after treatment, there was no significant difference between blank group A1 and Freund's A2 in the model group (P0.05), treatment group A3 rat peripheral tissues of adenosine increased significantly. With the control group, A1 There were significant differences between (P0.05), there was significant difference compared with the model group A2 (P0.05). Conclusion: 1. complete Freund's adjuvant is an antigen complex, can make the foot of the injection site of locally produced red, swollen and heat hyperalgesia behavior, and cause the change of histopathology, the the model is a common peripheral inflammatory pain model.2. manipulation of local analgesic effect significantly, can significantly improve the thermal pain threshold of.3. inflammatory pain in a rat model of manipulation therapy can increase local neurotransmitter adenosine concentration in inflammatory pain rats, tendon hand local analgesic effect by local adenosine release play a role.
