酸甘化阴法治疗肾性贫血的疗效观察

【摘要】：[目的]1.观察酸甘化阴法改善肾性贫血患者的临床疗效。2.探讨酸甘化阴法改善肾性贫血的中医作用机制。[方法]2015年11月至2016年12月,在黑龙江中医药大学附属第二医院肾病门诊选取符合纳入标准的慢性肾衰竭肾性贫血患者60例,随机分为治疗组30例,对照组30例。治疗组给予基础治疗同时服用中药经验方;对照组给予基础治疗同时服用益血生胶囊常规口服。疗程均为8周。观察两组患者治疗前、后的临床症状、血红蛋白、红细胞压积、血清白蛋白、超敏C-反应蛋白、血肌酐、尿素氮的变化情况。[结果]1.总体疗效比较:8周后,治疗组总有效率86.21%,对照组总有效率为64.29%,两组比较P0.05,差异有统计学意义,治疗组优于对照组。2.临床症状改善情况:8周后显示,治疗组与对照组比较,P0.05,差异有统计学意义;两组临床症状单项积分比较,气短懒言,神疲乏力,食少纳呆,腰膝酸软,脘腹胀满,P0.05,差异有统计学意义,治疗组优于对照组,表明酸甘化阴法对肾性贫血患者的临床症状具有明显的改善作用。3.Hb、HCT、ALB变化情况:8周后治疗组与对照组比较,P0.05,治疗组优于对照组,表明酸甘化阴法对肾性贫血及营养状态的改善作用。4.超敏C-反应蛋白变化情况:8周后显示,治疗组明显降低,与对照组比较,P0.05,差异有统计学意义,治疗组优于对照组,表明酸甘化阴法能够改善肾性贫血患者的微炎症状态。5.肾功能改善情况:SCr、BUN 8周后治疗组与对照组自身比较,P0.05,差异显著;治疗后,治疗组与对照组比较,P0.05,无显著性差异,表明酸甘化阴法对肾脏的保护作用与对照组相当。[结论]1.酸甘化阴法能够改善肾性贫血患者的临床症状;2.酸甘化阴法能够提高肾性贫血患者血红蛋白含量及红细胞压积,改善肾性贫血患者的营养状态;3.酸甘化阴法能够纠正肾性贫血患者的微炎症状态;4.酸甘化阴法对肾性贫血患者具有明显的肾脏保护作用,从而提高肾病患者的生活质量。
[Abstract]:[objective] 1.To observe the clinical curative effect of acidoglyceration of yin in improving renal anemia.To explore the mechanism of traditional Chinese medicine (TCM) in improving renal anemia by acid-glycosylation of yin method.[methods] from November 2015 to December 2016, 60 patients with chronic renal failure (CRF) were randomly divided into treatment group (n = 30) and control group (n = 30).The treatment group was treated with traditional Chinese medicine while the control group was given basic treatment and routine oral administration of Yixuesheng capsule.The course of treatment was 8 weeks.The changes of clinical symptoms, hemoglobin, hematocrit, serum albumin, hypersensitive C-reactive protein, serum creatinine and urea nitrogen were observed before and after treatment.[result] 1.The total effective rate of the treatment group was 86.21, the total effective rate of the control group was 64.29, the difference between the two groups was statistically significant, the treatment group was better than the control group.The difference was statistically significant. The treatment group was superior to the control group.The results showed that the acidoglyceration of yin method could obviously improve the clinical symptoms of patients with renal anemia. 3. The changes of HCT and ALB in the treatment group were significantly better than those in the control group after 8 weeks compared with the control group (P 0.05), and the treatment group was superior to the control group.The results showed that acidoglycan Yin method could improve renal anemia and nutritional status. 4.After 8 weeks, the changes of hypersensitive C-reactive protein in the treatment group were significantly lower than those in the control group (P 0.05). The treatment group was superior to the control group, indicating that the acid-glycosylated yin method could improve the microinflammation state of the patients with renal anemia.The improvement of renal function in the treatment group was significantly higher than that in the control group (P 0.05) after 8 weeks, but there was no significant difference between the treatment group and the control group (P 0.05), which indicated that the protective effect of acidoglycan Yin therapy on the kidney was equal to that in the control group.[conclusion] 1.Acidoglycan-Yin method can improve the clinical symptoms of renal anemia patients.Acidoglycan Yin method can improve hemoglobin content and hematocrit in patients with renal anemia and improve nutritional status of patients with renal anemia.Acidoglycan-Yin therapy can correct the microinflammatory state in patients with renal anemia.Acidoglycan-Yin method has obvious renal protection effect on renal anemia patients, thus improving the quality of life of patients with nephropathy.
