名老中医侯玉芬治疗闭塞性动脉硬化症的临证经验研究

【摘要】：目的:基于全国名老中医侯玉芬治疗闭塞性动脉硬化症(Arteriosclerosis obliterans,ASO)的疗效分析,初步总结和研究名老中医侯玉芬教授的思辨规律和用药特点,促进中医药治疗优势病种的临床研究和名老中医学术思想的传承。方法:筛选符合气虚血瘀型ASO研究标准的患者60例,并详细记录患者治疗前后中医证侯评分、踝肱比(Ankle-Brachial Index,ABI)、肢体远端血流灌注量和经皮氧分压以及血浆超敏C反应蛋白和红细胞沉降率。结果:60例患者经治疗后,结果显示:(1)临床症状和体征较治疗前均有明显改善,其显愈率为83.33%。(2)中医证候评分较治疗前均明显降低,具有非常显著性差异(P0.01)。(3)ABI较治疗前均明显升高,具有非常显著性差异(P0.01)。(4)肢体远端血流灌注量基线值、加热后血流灌注量以及血流灌注量增加率较治疗前均明显升高,具有非常显著性差异(P0.01)。(5)经皮氧分压较治疗前均明显升高,具有非常显著性差异(P0.01)。(6)血浆超敏C反应蛋白和红细胞沉降率较治疗前均明显降低,具有非常显著性差异(P0.01)。结论:名老中医侯玉芬诊治ASO,强调病症结合,内治与外治并举,善用补气活血药物。采用名老中医经验治疗气虚血瘀型ASO,可明显改善患者临床症状和患肢血运,同时可降低血浆超敏C反应蛋白和红细胞沉降率,减轻病变动脉的炎症反应,从而提高ASO患者的生活质量。
[Abstract]:Objective: based on the analysis of the curative effect of Hou Yufen in treating Arteriosclerosis obliterans ASO, we summarized and studied the law of thinking and the characteristics of drug use of Professor Hou Yufen, a famous old Chinese medicine, in the treatment of Arteriosclerosis obliterans.To promote the clinical research of traditional Chinese medicine (TCM) treatment of dominant diseases and the inheritance of famous Chinese medicine academic thought.Methods: 60 patients with Qi deficiency and blood stasis type ASO were selected and the scores of TCM syndromes before and after treatment were recorded in detail.Ankle-brachial Indextral ABI, distal limb blood perfusion, percutaneous partial pressure of oxygen, plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate.Results after treatment in 60 cases, the results showed that the clinical symptoms and signs were significantly improved, and the recovery rate was 83.33. 2) the scores of TCM syndromes were significantly lower than those before treatment, and the ABI was significantly higher than that before treatment (P 0.01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01, P < 0. 01).The baseline value of blood perfusion volume in distal extremity was significantly higher than that before treatment, and the blood perfusion volume and the increase rate of blood perfusion volume after heating were significantly higher than those before treatment, and the percutaneous partial pressure of oxygen was significantly higher than that before treatment.The plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate were significantly lower than those before treatment, and there was a very significant difference in plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate (P 0.01).Conclusion: Hou Yufen, a famous traditional Chinese medicine, emphasizes the combination of symptoms, internal treatment and external treatment, and makes good use of drugs for reinforcing qi and activating blood circulation.The treatment of Qi deficiency and blood stasis type ASO by the experience of famous old Chinese medicine can obviously improve the clinical symptoms and blood circulation of the affected limbs, at the same time, it can reduce the plasma hypersensitive C-reactive protein and erythrocyte sedimentation rate, and alleviate the inflammatory reaction of the diseased arteries.So as to improve the quality of life of patients with ASO.
