金匮肾气丸加味方和金匮肾气丸治疗糖尿病肾病Ⅳ期的临床对比研究

【摘要】：目的:本临床研究旨在对比金匮肾气丸原方及其加味方治疗糖尿病肾病IV期的临床疗效。方法:将入选的72例病例,按随机数表法分为治疗组36例,对照组36例。治疗组在基础治疗上予金匮肾气丸加味方,对照组在基础治疗上予金匮肾气丸原方,观察分析两组的中医症状及实验室指标变化情况,对比两组的临床疗效。结果:(1)治疗组与对照组总有效率分别为76.67%和46.67%,两组对比有显著性差异,治疗组明显优于对照组;(2)治疗组在改善中医症状方面明显优于对照组,两组相比差异显著;(3)两组自身治疗前后空腹血糖(FBG)、餐后2小时血糖(PBG)、糖化血红蛋白(HbA1c)均有明显下降,治疗后两组餐后2小时血糖(PBG)相比无统计学意义,治疗后治疗组FBG下降优于对照组,具有统计学意义,治疗后治疗组HbA1c下降明显优于对照组,具有极显著性差异;(4)两组治疗后24h尿蛋白定量与治疗前相比有所改善,组间对比亦具统计学意义;(5)治疗后两组血尿素氮(BUN)、血肌酐(Scr)水平与治疗前比较均有明显下降,且治疗组下降趋势明显优于对照组,治疗后两组血内生肌酐清除率(Ccr)较治疗前明显均上升,组间对比亦具极显著性差异。结论:金匮肾气丸加味方与原方相比,可更明显改善糖尿病肾病IV期患者临床症状,维持血糖稳定,减少尿蛋白,改善水肿情况,延缓肾功能恶化。
[Abstract]:Objective: to compare the clinical effect of Jinkui Shenqi Pill and its Jiawei prescription on stage IV diabetic nephropathy.Methods: 72 cases were randomly divided into treatment group (36 cases) and control group (36 cases).The treatment group was treated with Jinkui Shenqi Pill on basic treatment and the control group with Jinkui Shenqi Pill on basic treatment. The changes of TCM symptoms and laboratory indexes in the two groups were observed and analyzed, and the clinical efficacy of the two groups was compared.Results the total effective rates of the treatment group and the control group were 76.67% and 46.67%, respectively. There was significant difference between the two groups, and the treatment group was obviously superior to the control group in improving the symptoms of traditional Chinese medicine.There was a significant difference between the two groups (P < 0.05). The fasting blood glucose (FBGG), blood glucose (PBG) and HbA1c (2 hours postprandial blood glucose) in the two groups were significantly decreased before and after treatment, but there was no significant difference between the two groups after treatment.After treatment, the decrease of FBG in the treatment group was better than that in the control group, and the decrease of HbA1c in the treatment group was significantly better than that in the control group.The blood urea nitrogen bun and creatinine (SCR) levels in the two groups were significantly lower than those before the treatment, and the decreasing trend of the treatment group was obviously better than that of the control group.After treatment, the creatinine clearance rate (CcrR) in the two groups was significantly higher than that before treatment, and there was also a significant difference between the two groups.Conclusion: compared with the original prescription, Jingui Shenqi Pill can significantly improve the clinical symptoms of patients with stage IV diabetic nephropathy, maintain stable blood glucose, reduce urine protein, improve edema and delay the deterioration of renal function.
