益气健脾和胃法治疗慢性萎缩性胃炎（脾胃虚弱证）的临床观察

【摘要】：目的:观察运用益气健脾和胃法治疗慢性萎缩性胃炎脾胃虚弱证患者的临床症状、胃镜下黏膜及病理的改善情况,以探究其临床疗效。方法:本次临床观察采用治疗前后自身对照的方式进行研究。选择46例慢性萎缩性胃炎且中医辨证为脾胃虚弱证的患者入组,均予以谢晶日教授益气健脾和胃法的中药汤剂。每日早晚各150毫升温服,服药时间均为饭后半小时。规范治疗8周后,通过患者自身前后积分对照评价治疗效果,并利用SPSS17.0统计软件进行数据分析。结果:1.治疗8周后,在胃脘胀满、胃痛、喜温喜按、纳呆少食、大便稀溏、气短乏力方面,中医证候积分治疗前后均有显著统计学差异(P0.01)。而口淡积分治疗前后无统计学差异(P=0.3170.01)。中医证候治疗总有效率为:97.5%。2.胃镜下积分治疗前后有显著差异(P=0.0000.01),总有效率为:37.8%。3.治疗前后,黏膜萎缩积分有显著差异(P=0.0000.01),总有效率为47.5%。在肠化积分方面,治疗前后有显著差异(P=0.0030.01),总有效率为20.5%。而治疗前后上皮内瘤变积分无统计学差异(P=0.1570.01),总有效率为 33.3%。结论:1.益气健脾和胃法能有效改善慢性萎缩性胃炎脾胃虚弱证患者的中医症状。2.益气健脾和胃法能在一定程度上改善慢性萎缩性胃炎脾胃虚弱证患者胃镜下黏膜情况及病理表现。
[Abstract]:Objective: to observe the improvement of clinical symptoms, mucosal and pathological changes in patients with chronic atrophic gastritis with deficiency of spleen and stomach by invigorating qi and invigorating spleen and stomach. Methods: the clinical observation was conducted by self-control before and after treatment. Forty-six patients with chronic atrophic gastritis and TCM syndrome differentiation as spleen and stomach deficiency syndrome were selected into the group, and all the patients were given traditional Chinese medicine decoction of Yiqi Jianpi and stomach method by Professor Xie Jingri. Daily morning and evening 150 ml warm take, the time of taking medicine is half an hour after meals. After 8 weeks of standardized treatment, the therapeutic effect was evaluated by the control of the patients before and after the treatment, and the data were analyzed by SPSS17.0 software. The result is 1: 1. After 8 weeks of treatment, there were significant statistical differences before and after treatment in the aspects of stomachache, stomachache, warm and happy press, eating less food, loose stool, asthenia of breath, and the integral of TCM syndromes before and after treatment. However, there was no statistical difference before and after treatment. The total effective rate of TCM syndrome treatment is: 97.5. There was significant difference between the two groups before and after the treatment, and the total effective rate was 0.0000.01g. The total effective rate was: 37.8%. There was significant difference in mucosal atrophy score before and after treatment. The total effective rate was 47.5%. There was significant difference in intestinal metaplasia score before and after treatment, and the total effective rate was 20.5. However, there was no statistical difference in intraepithelial neoplasia score before and after treatment, and the total effective rate was 33.3b. Conclusion 1. Invigorating qi and invigorating spleen and stomach can effectively improve TCM symptoms of chronic atrophic gastritis with deficiency of spleen and stomach. The method of invigorating qi and invigorating spleen and stomach can improve the mucosal condition and pathological manifestation of patients with chronic atrophic gastritis with deficiency of spleen and stomach to a certain extent.
