益气滋阴通络方治疗气阴两虚、脉络瘀阻型2型糖尿病周围神经病变的临床观察

【摘要】：目的:观察益气滋阴通络方治疗气阴两虚、脉络瘀阻型2型糖尿病周围神经病变的临床疗效。方法:采用随机、平行对照的临床试验研究方法,将符合研究要求的2型糖尿病神经病变的患者60例采用数字表法随机分为对照组(30例)与治疗组(30例),两组患者糖尿病基础治疗不变,对照组给予弥可保常规治疗,治疗组在弥可保常规治疗基础上给予益气滋阴通络方一日一剂治疗。治疗4周后,观察血脂,血糖,血流变,密歇根筛查量表评分,周围神经传导速度,血清8-异前列腺素F2α(8-iso-PGF-2α)及治疗前后中医临床症候疗效的变化,评价益气滋阴通络方的临床疗效。结果:1.中医临床症候疗效:治疗组总有效率93.3%,对照组总有效率60.0%,组间比较具有显著性差异(P0.05);2.密歇根筛查量表评分:两组患者治疗前后密歇根筛查量表评分均有明显下降(P0.05),两组治疗前后评分差值比较具有显著的差异(P0.05),提示治疗组对密歇根筛查量表评分的改善优于对照组;3.血清8-iso-PGF-2α:两组患者经药物治疗后均可显著降低血清8-iso-PGF-2α水平(P0.05),但治疗前后8-iso-PGF-2α差值治疗组较对照组具有显著的差异(P0.05),提示治疗组对8-iso-PGF-2α的改善优于对照组;4.周围神经传导速度:治疗前后相比治疗组周围神经传导速度显著降低(P0.05),对照组无显著变化(P0.05);5.治疗组在改善血脂、血糖、血流变方面显著优于对照组(P0.05)。结论:益气滋阴通络方对气阴两虚、脉络瘀阻型糖尿病周围神经病变患者血脂、血糖、血流变、密歇根筛查量表评分、8-iso-PGF-2α、周围神经传导速度有改善作用,并可明显改善DPN患者中医临床症状,提高生活质量,验证了采用益气滋阴通络方治疗气阴两虚、脉络瘀阻型DPN的可行性,为中医药防治DPN提供了一种的新的思路与方法。
[Abstract]:Objective: to observe the clinical effect of Yiqi Ziyin Tongluo recipe on peripheral neuropathy of type 2 diabetes mellitus with deficiency of qi and yin and choroid stasis. Methods: a randomized, parallel controlled clinical trial was conducted. 60 patients with type 2 diabetic neuropathy were randomly divided into control group (n = 30) and treatment group (n = 30). The treatment group was treated with Yiqi Ziyin Tongluo prescription on the basis of routine treatment. After 4 weeks of treatment, the changes of blood lipids, blood glucose, hemorheology, Michigan screening scale, peripheral nerve conduction velocity, serum 8-iso-PGF-2 伪) and the clinical symptoms before and after treatment were observed. To evaluate the clinical effect of Yiqi Ziyin Tongluo recipe. The result is 1: 1. The curative effect of TCM clinical symptoms: the total effective rate of the treatment group was 93.3m, the total effective rate of the control group was 60.0.The difference between the two groups was significant (P 0.05). Michigan screening scale score: the scores of Michigan screening scale were significantly decreased in both groups before and after treatment, and there was significant difference between the two groups before and after treatment (P 0.05), indicating that the Michigan screening scale score in the treatment group was higher than that in the control group. The improvement was better than that of the control group. Serum 8-iso-PGF-2 伪: after drug treatment, the serum 8-iso-PGF-2 伪 level was significantly decreased in both groups, but the difference of 8-iso-PGF-2 伪 between the treatment group and the control group was significant (P 0.05), indicating that the improvement of 8-iso-PGF-2 伪 in the treatment group was better than that in the control group (P < 0.05). Peripheral nerve conduction velocity: compared with the treatment group, the peripheral nerve conduction velocity of the treatment group was significantly lower than that of the control group, while the control group had no significant change. The treatment group was superior to the control group in improving blood lipids, blood glucose and hemorheology. Conclusion: Yiqi Ziyin Tongluo recipe can improve blood lipid, blood glucose, blood rheology, 8-iso-PGF-2 伪 and peripheral nerve conduction velocity in diabetic patients with deficiency of qi and yin and choroidal stasis. It can obviously improve the clinical symptoms of traditional Chinese medicine (TCM) and improve the quality of life in patients with DPN. It verifies the feasibility of using Yiqi Ziyin Tongluo recipe to treat DPN of deficiency of Qi and Yin and of choroid stasis, and provides a new way of thinking and method for the prevention and treatment of DPN by traditional Chinese medicine.
