咽五针治疗脑卒中后吞咽困难的临床研究

【摘要】：目的观察咽五针治疗脑卒中后吞咽困难的临床疗效。方法将180例脑卒中后吞咽困难患者随机分为基础治疗组,传统针刺组和针刺咽五针组,每组各60例。基础治疗组进行常规的基础治疗,传统针刺组在基础治疗组治疗基础上取传统针刺穴位进行治疗,针刺咽五针组在基础治疗组治疗基础上取治呛穴、吞咽穴、提咽穴、发音穴、治返流穴进行治疗,疗程均为60d。以标准吞咽功能评分量表(SSA)、电视荧光吞咽检查(VFSS)和中风后吞咽困难的中医辨证量表进行疗效评定。结果治疗后针刺咽五针组总有效率93.3%(56/60),显著高于基础治疗组68.3%(41/60,P0.01)和传统针刺组的81.7%(49/60,P0.05);针刺咽五针组SSA、VFSS和中风后吞咽困难的中医辨证量表评分较基础治疗组和传统针刺组改善更为显著(均P0.05)。结论针刺咽五针组能有效改善脑卒中后吞咽困难。
[Abstract]:Objective to observe the clinical effect of five-needle pharynx on dysphagia after stroke. Methods 180 patients with dysphagia after stroke were randomly divided into basic treatment group, traditional acupuncture group and five-needle acupuncture group with 60 cases in each group. The basic treatment group was treated with routine basic treatment, the traditional acupuncture group took the traditional acupuncture points for treatment on the basis of the basic treatment group, and the acupuncture five-needle group treated the choking points, swallowing the points, lifting the pharynx points, and the pronunciation points on the basis of the treatment of the basic treatment group. The treatment course was 60 days. The therapeutic effects were evaluated by standard swallowing score scale (SSAA), TV fluorescence swallowing examination (VFSS) and TCM syndrome differentiation scale for dysphagia after stroke. Results after treatment, the total effective rate of the five-needle acupuncture group was 93.3% 56 / 60%, which was significantly higher than that of the basic treatment group (68.3%) and the traditional acupuncture group (81.7% / 60% P0.05); the SSAVFSS of the five-needle acupuncture group and the TCM syndrome differentiation scale score of dysphagia after stroke were higher than those of the basic treatment group and the traditional acupuncture group. The improvement was more significant (P 0.05). Conclusion Acupuncture can effectively improve dysphagia after stroke.
