基于Meta分析的益气养阴法治疗功能性便秘的系统评价

【摘要】：目的:通过运用Cochrane系统评价方法,客观评价益气养阴法治疗功能性便秘的临床疗效,揭示该法治疗功能性便秘的优势,为临床治疗方法的选择及应用提供研究证据。方法:详细搜集1995.12-2015.12国内公开发表的关于益气养阴法治疗功能性便秘的研究文献,严格按照纳入标准和排除标准择取相关文献,纳入以中医益气养阴法治疗为试验组,以西医治疗(PEG、促动力药、酚酞片等口服)为对照组的试验研究,评价选取的文献质量,对所需详细相关信息予以提炼。Meta分析选用最新Review Manager5.3软件,分析综合疗效、症状积分、复发率及不良反应,描绘森林图、检验敏感性、分析漏斗图。结果:1.共纳入符合标准的37篇文献进行研究,共计3565例功能性便秘患者;2.大部分纳入文献研究的方法学质量偏低,4-7分的仅有1篇,12篇3分文献,20篇2分文献,1分的有4篇;3.试验组中药使用频率统计显示:主要为益气养阴类药物(黄芪、白术、生地黄、麦冬、当归、玄参等);4.Meta分析结果:除在减少不良反应方面,中医益气养阴法与西医治疗无明显差异外,在综合疗效、改善症状、降低复发率等方面中医益气养阴法治疗FC优势明显。结论:本研究对近20年中医药益气养阴法治疗功能性便秘的状况进行了客观评价,结果表明,该法治疗功能性便秘在综合疗效、改善症状及降低复发率方面效果显著。由于符合此次研究的文献质量评价普遍较低,导致发表偏倚,期待以后能有质量高、样本大的临床试验对结果的可靠性加以佐证,促进中医药的理论与实践相结合。
[Abstract]:Objective: to objectively evaluate the clinical efficacy of the method of supplementing qi and nourishing yin in the treatment of functional constipation by using Cochrane system evaluation method, and to reveal the advantages of this method in the treatment of functional constipation, and to provide evidence for the selection and application of clinical treatment methods. Methods: the literature on the treatment of functional constipation by invigorating qi and nourishing yin was collected in detail from May 12 to May 12, 2015.The relevant documents were selected strictly according to the inclusion standard and exclusion standard, and were included in the experimental group treated with traditional Chinese medicine to nourish qi and nourish yin. The control group was treated with western medicine as control group. The quality of the selected literature was evaluated, and the relevant information was extracted. The latest Review Manager5.3 software was used to analyze the comprehensive curative effect and symptom score. Recurrence rate and adverse reactions, forest map, sensitivity test, funnel map analysis. The result is 1: 1. A total of 3565 functional constipation patients were studied in 37 articles. In most of the literature studies, the quality of methodology was relatively low (4-7). Only one of 12 papers with 3 points was divided into 20 papers with 2 points, and 4 articles with 1 score were classified into 4 papers and 3 papers respectively. Statistics of the frequency of Chinese medicine used in the experimental group showed that the main drugs (Astragalus membranaceus, Atractylodes macrocephala, Radix Rehmanniae, Radix Ophiopogonis, Radix angelicae Sinensis, Radix Hyacinopsis et al.). 4. Meta-analysis results: except for the reduction of adverse reactions, There is no significant difference between traditional Chinese medicine and western medicine in the comprehensive efficacy, improve symptoms, reduce recurrence rate and other aspects of traditional Chinese medicine Qi nourishing yin treatment of FC advantages. Conclusion: this study made an objective evaluation on the treatment of functional constipation with traditional Chinese medicine in the past 20 years. The results showed that this method had remarkable effect in treating functional constipation, improving symptoms and reducing recurrence rate. Because the evaluation of literature quality according with this study is generally low, it leads to publication bias. It is expected that there will be high quality and large sample clinical trials to prove the reliability of the results in order to promote the combination of theory and practice of traditional Chinese medicine.
