针灸对老年脑卒中后痉挛性偏瘫患者肢体运动功能、日常生活活动能力的影响

【摘要】：目的探讨老年脑卒中后痉挛性偏瘫患者针灸治疗的临床效果及肢体运动功能Fugl-Meyer(FMA)评分和日常生活活动改良Barthel指数(BI)变化。方法 94例老年脑卒中后痉挛性偏瘫患者随机分为观察组和对照组各47例。两组均给予常规基础药物治疗,观察组在此基础上给予针灸治疗,随访3个月。用改良Ashworth痉挛分级量表对治疗前后痉挛程度进行评价,测定FMA评分和BI指数。结果治疗前两组痉挛分级无显著差异(P0.05),治疗后观察组痉挛分级显著优于对照组(P0.05);治疗前两组FMA和BI评分无显著差异(P0.05),治疗后1、2、3个月观察组FMA和BI评分均显著优于对照组(P0.05)。结论常规治疗基础上给予老年脑卒中后痉挛性偏瘫患者针灸治疗有助于减轻痉挛症状,同时对患者肢体运动能力和生活能力也有显著改善作用。
[Abstract]:Objective to investigate the clinical effect of acupuncture and moxibustion in elderly patients with spastic hemiplegia after stroke and the changes of Fugl-Meyer 's FMA score and modified Barthel index. Methods 94 elderly patients with spastic hemiplegia after stroke were randomly divided into observation group (n = 47) and control group (n = 47). Both groups were treated with routine basic drugs, and the observation group were treated with acupuncture and moxibustion for 3 months. The degree of spasticity was evaluated by modified Ashworth scale before and after treatment. FMA score and BI index were measured. Results before treatment, there was no significant difference in spasticity grading between the two groups (P 0.05), but the observation group was significantly better than the control group (P 0.05) in the spasm grading before treatment, and there was no significant difference in the FMA and BI scores between the two groups before treatment (P 0.05). The FMA and BI scores in the observation group were significantly better than those in the control group at 12 and 3 months after treatment. Conclusion Acupuncture and moxibustion therapy on the basis of routine treatment for the elderly patients with spastic hemiplegia after stroke is helpful to alleviate the symptoms of spasticity and improve the motor ability and life ability of the patients.
