近二十年针刺治疗紧张性头痛的选穴规律探析

【摘要】：目的:搜集近二十年国内数据库中收录的有关针刺治疗紧张性头痛的文章,运用云数据技术,探析取穴组方规律,为相关科研与临床工作提供依据。方法:本研究以中国知识资源总库、万方、维普数据库为检索源,以1996.06~2015.05为期限,以“主题=紧张性头痛并且主题=针刺或腧穴”为条件,利用计算机统计取穴、方法、疗程频次,并探索其规律性。结果:1.主穴的统计情况:针刺治疗紧张性头痛的主穴共69个,涉及全身各个部位,分布在头部的有32个,四肢部共25个,以风池、百会、太阳穴运用频率最高。2.配穴的统计情况:按疼痛部位选取的配穴多以原穴、五腧穴和局部选穴为主;按证型配穴中,较常见的致病因素与肝郁、痰湿、瘀血、气血的关系最密切,根据证型选取的配穴以脾经、胃经腧穴居多。3.针刺治疗常结合使用的方法有16种,以电针、中药、放血疗法应用频率最高。4.治疗时间的制定,多数为留针30分钟,一日一次,共20日;重要腧穴注重针刺深度与方向,风池穴向鼻尖方向斜刺较为安全,刺入0.8~1.2寸,百会穴多平刺,刺入0.5~0.8寸。结论:针刺治疗紧张性头痛选穴组方原则明确,注重辨证配穴,可结合多种方法治疗,效果显著。组方以止痛安神,调和气血为原则,选穴以头部配合四肢远端取穴为主,配合循经或辨证取穴。常配合电针、刺血等法,加强镇痛效果。留针时间与疗程需综合病情制定。重要穴位注重针刺深度,方向据病位选择。
