清热活血法治疗冠心病不稳定性心绞痛临床疗效的Meta分析

【摘要】：目的:本研究通过Meta分析客观的评价运用清热活血法治疗冠心病不稳定性心绞痛的临床疗效,为清热活血类中药在临床推广应用提供循证医学依据。方法:根据制定科学的检索策略,严格限定纳入标准与排除标准,运用电子计算机检索或手工检索的方法检索各数据库从开始建立时至2016年10月期间已发表的运用清热活血法治疗冠心病不稳定性心绞痛临床疗效的相关文献,并采用Jadad量表对所筛选的文献进行质量的评价,通过专用软件对收集的文献里包含的数据进行Meta分析和异质性检验。结果:最终纳入本研究的文献共19篇,发表时间为2004-2016年,共累计1576例患者。经Meta分析得知:临床心绞痛症状的改善OR=3.65,95�jCI[2.70 4.93],P0.00001;临床中医证候的改善OR=4.29,95�jCI[2.66 6.92],P0.00001;临床心电图表现的改善OR=2.53,95�jCI[1.81 3.53],P0.00001;临床C反应蛋白的改善WMD=-1.53,95�jCI[-2.35,-0.72],P=0.0002;临床总胆固醇的改善WMD=-0.65,95�jCI[-1.16-0.14],P=0.01;低密度脂蛋白的改善WMD=-0.31,95�jCI[-0.52-0.09],P=0.005。结论:应用清热活血法治疗冠心病不稳定性心绞痛时能有效的缓解临床心绞痛症状,改善中医证候,改善心电图表现,降低血清炎症因子水平及血脂水平,其临床疗效确切,是治疗冠心病不稳定性心绞痛有希望的选择。
[Abstract]:Objective: to evaluate the clinical efficacy of clearing heat and activating blood circulation method in treating unstable angina pectoris of coronary heart disease by Meta analysis, and to provide evidence based medicine basis for the clinical application of traditional Chinese medicine for clearing heat and activating blood circulation. Methods: according to the scientific retrieval strategy, the inclusion criteria and exclusion criteria were strictly limited. The relevant literatures on the clinical efficacy of clearing heat and activating blood circulation in the treatment of unstable angina pectoris of coronary heart disease were searched by computer search or manual retrieval from the beginning of establishment to October 2016. Jadad scale was used to evaluate the quality of the selected documents, and the data contained in the collected documents were analyzed by Meta and heterogeneity test with special software.
