腹部推拿治疗心脾两虚型广泛性焦虑症的随机对照研究

【摘要】：目的:评价腹部推拿治疗心脾两虚型广泛性焦虑症的临床疗效。方法:将符合心脾两虚型广泛性焦虑症纳入标准的60例患者,按随机数字表法分为试验组和对照组各30例。试验组给予腹部推拿治疗,每日治疗1次,每次40min;对照组给予口服丁螺环酮5 mg,每日3次;两组均15 d为1个疗程,治疗2个疗程。分别对两组患者治疗前后血浆五羟色胺(5-HT)含量及临床症状进行比较,并对患者的治疗前、治疗后及随访时(疗程结束后3个月)的焦虑自评量表(SAS)和汉密尔顿焦虑量表(HAMA)评分进行比较和评价。结果:试验组总有效率92.59%,愈显率55.56%;对照组总有效率86.96%,愈显率52.2%,两组比较无显著性差异(P0.05)。两组疗法对于改善紧张不安、失眠健忘两种症状的临床疗效无显著性差异(P0.05),而对于改善腹胀便溏症状方面,试验组则明显优于对照组(P0.05)。两组患者治疗30 d后,5-HT含量、SAS评分以及HAMA评分均较治疗前显著下降(P0.05),且两组间比较无明显差异(P0.05)。治疗后3个月随访结果显示,试验组SAS评分及HAMA评分均无明显上升(P0.05);而对照组SAS评分及HAMA评分较治疗前均有明显反弹(P0.05)。不良反应分析方面,试验组患者无不良反应出现;对照组4例出现不良反应,不良反应发生率为13.33%。结论:腹部推拿可有效治疗心脾两虚型广泛性焦虑症,并具有操作简便、无创、无不良反应、疗效稳定持久等优势,值得临床进一步研究推广。
[Abstract]:Objective: to evaluate the clinical effect of abdominal massage on generalized anxiety disorder with deficiency of heart and spleen. Methods: 60 patients with generalized anxiety disorder with deficiency of heart and spleen were divided into two groups according to random digital table: the experimental group (n = 30) and the control group (n = 30). The experimental group was treated with abdominal massage once a day for 40 minutes, while the control group was given oral bupirone 5 mg 3 times a day, both 15 days as a course of treatment, 2 courses of treatment. Plasma serotonin 5-HT levels and clinical symptoms were compared between the two groups before and after treatment. The scores of self-rating anxiety scale (SAS) and Hamilton anxiety scale (Hama) were compared and evaluated after treatment and follow-up (3 months after the end of the course of treatment). Results: the total effective rate of the test group was 92.59%, the more obvious rate was 55.56%, the total effective rate of the control group was 86.96%, the more obvious rate was 52.2%, there was no significant difference between the two groups (P 0.05). There was no significant difference in clinical effect between the two groups in improving tension and restlessness, and there was no significant difference in the clinical effect of insomnia and amnesia. However, the experimental group was better than the control group in improving the symptoms of loose stools. After 30 days of treatment, the scores of 5-HT and HAMA in both groups were significantly lower than those before treatment (P 0.05), and there was no significant difference between the two groups (P 0.05). The results of follow-up 3 months after treatment showed that there was no significant increase in SAS score and HAMA score in the trial group, while the SAS score and HAMA score in the control group were significantly higher than those before treatment. There were no adverse reactions in the test group and 4 cases in the control group, with an adverse reaction rate of 13.33%. Conclusion: abdominal massage can effectively treat generalized anxiety disorder with deficiency of heart and spleen, and has the advantages of simple operation, non-invasive, no adverse reaction, stable and lasting curative effect and so on, which is worthy of further clinical study and promotion.
