基于数据分析探讨张鸣鹤教授治疗活动期关节炎的用药规律

【摘要】:本研究由两部分组成,首先通过深化理论认识,探索研究路线。然后通过整理收集由张鸣鹤教授诊治且收获良效的风湿病活动期关节炎患者的病历资料,以中医传承软件作为技术支持,对众多病历的处方数据进行相关关系分析,获取疾病治疗的核心药物,在此基础上通过以方测证、有是证用是药等原理,进行处方数据的深层次挖掘,并结合文献查阅及既往侍诊中所记录的张老用药经验,对张老治疗部分活动期关节炎的学术观点、辨证思路及用药经验作出分析整理,由知其然过渡至知其所以然,深化理论及用药认识,以进一步指导临床学习。通过本研究的探讨,加深了对中西医优势相参诊疗疾病的认识,即将中医辨证施治的个体化思路与西医认识疾病的客观规范化思路相结合,以探寻疾病诊疗的最优方案。因处风湿病活动期的关节炎,具有炎热毒邪致痹的相同病理损害,故以清热解毒作为治疗大法;据本病病因多为风寒湿邪,且久病邪深,需给邪以出路,故重用祛风除湿药,以驱表达邪。因此风湿病活动期关节炎的治疗以清热解毒药联用祛风除湿药为主。除此之外,治疗需依据个体差异、病气之偏胜分而论治,并通过对用药原则及药物性能的灵活把握,选用具有清热解毒、祛风除湿、止痹痛、通经络、活血行血、化瘀散结、利水消肿等数效兼备之药,配伍组方,确保用药精准,疗效可靠。
[Abstract]:This research consists of two parts. Firstly, the research route is explored by deepening the theoretical understanding. Then through collating and collecting the medical records of active arthritis patients with rheumatism treated and treated by Professor Zhang Minghe, taking the traditional Chinese medicine inheritance software as the technical support, the correlation analysis of prescribing data of many medical records is carried out. On the basis of obtaining the core drugs for the treatment of the disease, and on the basis of testing the evidence by the prescription, including the principle of whether the syndrome is used or the medicine is used, to carry out the deep-level mining of the prescription data, and combining with the experience of Zhang Lao's medication recorded in the literature review and previous consultations, In order to further guide clinical study, Zhang Lao's academic viewpoint on treating some active arthritis, dialectical thinking and medication experience were analyzed and sorted out, from knowing its nature to knowing its reason, deepening its theory and understanding of drug use. Through the discussion of this study, it deepens the understanding of the advantages of TCM and Western medicine in the diagnosis and treatment of diseases, that is, the combination of individualized thinking of TCM dialectical treatment and objective and standardized thinking of western medicine understanding of diseases, in order to explore the best scheme of diagnosis and treatment of diseases. Because arthritis in the active phase of rheumatism has the same pathological damage as arthralgia caused by hot toxin evil, it uses clearing heat and detoxification as the major treatment method. According to the etiology of this disease, most of them are wind, cold and dampness evil, and the long illness is deep, so it is necessary to give the evil a way out, so it is necessary to reuse the medicine for dispelling wind and removing dampness. To exorcise evil spirits. Therefore, rheumatism active arthritis treatment with heat-clearing and detoxification combined with dispelling wind and dehumidification drugs. In addition, the treatment should be based on individual differences, the partial victory of disease and qi, and through the flexible grasp of the principles of drug use and the performance of the drug, it should be selected to have the characteristics of clearing heat and detoxifying, dispelling wind and removing dampness, stopping arthralgia, opening channels and collaterals, activating blood circulation, and resolving blood stasis and dispersing knot. Lishui, detumescence and other effective drugs, compatible with the prescription, to ensure that the drug is accurate, effective and reliable.
