针灸治疗功能性便秘选穴规律的文献研究

【摘要】：目的:本课题通过对近十年针灸治疗功能性便秘的临床随机对照试验进行整理、总结及统计分析,探讨针灸治疗功能性便秘的选穴规律,为探寻有效的针灸治疗功能性便秘的取穴处方提供参考。方法:电子检索相关期刊论文(CNKI),维普期刊数据库(VIP)及万方数据库,检索词为"针"、"针刺"、"针灸""功能性便秘","便秘",检索年限设定为2005年一月至2016年12月。获得全文后文献统一用noteexpress进行管理,浏览文献,根据纳入标准及排除标准筛选出符合纳入条件的文献,对文献中使用的腧穴、腧穴归经、腧穴所属身体部位等。以及特定穴使用情况进行,统计学处理将信息导入电子表格进行分析和统计计算,并进行计算次数统计论述分析。对个体数据采用频数分析进行统计学分析,用spss20.0统计分析软件进行计数数据的统计描述分析其规律。结果:按照本研究的检索策略,在中国期刊网、万方期刊数据库及维普数据库中共检索出文献578篇,排除重复性文献、个案报道、综述类文献及其他文献,最终纳入文献65篇。从2005年1月至2016年12月,临床上用于治疗功能性便秘的穴位共51个,总穴次594次。根据统计结果显示,排名前十的穴位分别是天枢、上巨虚、足三里、大肠俞、支沟、气海、关元、中脘、腹结、肾俞。其中28篇是针刺疗法,5篇是穴位注射疗法。7篇是穴位埋线疗法,其余25篇是综合疗法。临床上常用于治疗功能性便秘的经脉主要包括足阳明胃经(25.49%)、足太阳膀胱经(21.57%)、任脉(13.73%)、足太阴脾经(11.76%)、手阳明大肠经(9.80%),占总数的82.35%。特定穴中,以背俞穴及络穴使用最多,其次为募穴、合穴及下合穴。将所统计出的51个腧穴按人体部位划分,可得表2,观察发现临床多运用腹部穴位及下肢部穴位运用最多,上肢穴位及腰骶部穴位次之。最次才是胸背部足太阳膀胱经穴位。由此可见,针灸治疗功能性便秘以腹部选穴为主,结合四肢部选穴为辅。结论:现代临床研究针灸治疗功能性便秘的选穴规律:以足阳明经取穴为主,辅以任脉及足太阳膀胱经腧穴。针刺治疗取穴方面,以腹部选穴为主,搭配循经四肢部取特定穴。腧穴分布以腹部和下肢部穴位为主,上肢部穴位次之。
[Abstract]:Objective: to summarize and analyze the clinical randomized controlled trials of acupuncture and moxibustion treatment of functional constipation in the past ten years, and to explore the rule of acupoint selection in the treatment of functional constipation by acupuncture and moxibustion. To explore effective acupuncture and moxibustion treatment of functional constipation point prescription to provide a reference. Methods: Chinese Journal Full-text Database (CNKI), Weip Journal Database (VIP) and Wanfang Database were searched electronically. The key words were "acupuncture", "functional constipation" and "constipation". The searching period was set from January 2005 to December 2016. After obtaining the full text, the literature was managed by noteexpress, and the literature was browsed. According to the inclusion standard and the exclusion standard, the literature suitable for inclusion was selected, and the acupoints, body parts and so on were used in the literature. And the use of specific holes, statistical processing information into the spreadsheet for analysis and statistical calculation, and statistical analysis of the number of calculations. The frequency analysis is used to analyze the individual data, and the statistical description of the counting data is analyzed by spss20.0 software. Results: according to the retrieval strategy of this study, 578 articles were retrieved in China Journal Network, Wanfang Journal Database and Weip Database, excluding repetitive literature, case reports, review literature and other literature, and 65 articles were included. From January 2005 to December 2016, 51 points were used to treat functional constipation, with a total of 594 points. According to the statistical results, the top ten acupoints were Tianshu, Shangjuxu, Zusanli, Dachang Yu, branchou, Qihai, Guan Yuan, Zhongwan, abdominal knot, Shenshu. Among them, 28 were acupuncture therapy, 5 were acupoint injection therapy, .7 were acupoint catgut embedding therapy, and the other 25 were combined therapy. The main meridians commonly used in the treatment of functional constipation include the stomach Meridian of foot Yangming (25.49%), bladder Meridian of foot Sun (21.57%), Renmai (13.73%), spleen Meridian of foot Taiyin (11.76%) and large intestine Meridian of hand Yangming (9.80%), accounting for 82.35 points of the total. Among the specific points, the points of back-Shu and collaterals were used the most, followed by the points of Jun, he and Xianhe. The 51 acupoints were divided according to human body position, and table 2 was obtained. It was found that abdominal acupoints and lower extremity acupoints were used most frequently in clinic, followed by upper limb acupoints and lumbosacral points. The most important is the chest, back, foot, sun, bladder meridian. Thus, acupuncture and moxibustion treatment functional constipation in the abdominal point selection, combined with the extremities point selection as a supplementary. Conclusion: in modern clinical study, the rule of acupoint selection in the treatment of functional constipation by acupuncture and moxibustion is as follows: the acupoints of the Meridian of foot Yang Ming and the acupoint of bladder Meridian of Renmai and foot Sun. Acupuncture treatment for acupoints, the abdominal selection of the main point, with the limbs along the meridian to take specific points. The distribution of acupoints was mainly abdominal and lower limb acupoints, followed by upper limb acupoints.
