补肾解郁汤治疗老年高血压病伴抑郁状态患者的临床研究

【摘要】：目的:观察并评价补肾解郁汤治疗老年高血压病伴抑郁状态患者的临床疗效。方法:根据纳入和排除标准筛选老年高血压病伴抑郁状态患者60例,随机分为治疗组及对照组各30例。两组患者均给予常规西药降压治疗,治疗组加服中药补肾解郁汤,每日一剂,早晚餐后半小时各服一次,疗程均为4周。观察两组患者治疗后总疗效;治疗前后中医症候积分、中医病情分级、血压、汉密尔顿抑郁量表(HAMD)评分、老年抑郁量表(GDS)评分、血浆脑源性神经营养因子(BDNF)、P物质的变化,并做统计学处理。结果:治疗组治疗后中医证候总有效率93.33%,与对照组73.33%相比,具有显著性差异(P0.05);治疗组治疗后中医病情分级明显改善(P0.05),且优于对照组;治疗组收缩压和舒张压均降至正常范围,治疗组优于对照组(P0.05);治疗组治疗后降压疗效总有效率93.33%,对照组83.4%,具有显著性差异(P0.05);治疗组治疗后汉密尔顿抑郁量表(HAMD)、老年抑郁量表(GDS)评分均明显下降(P0.01),且优于对照组;治疗组治疗后血浆脑源性神经营养因子(BDNF)较治疗前明显升高(P0.05)、P物质较治疗前明显降低(P0.05),与对照组比较,具有明显的差异。结论:补肾解郁汤能提高老年高血压病伴抑郁状态患者的疗效,改善中医症状,降低中医病情分级,改善血压水平,降低HAMD、GDS量表评分,调节BDNF、P物质水平,治疗老年高血压病伴抑郁状态患者的疗效确切,在临床上值得更进一步的推广和运用。
[Abstract]:Objective: to observe and evaluate the clinical effect of Bushen Jieyu decoction on elderly patients with hypertension and depression. Methods: according to the criteria of inclusion and exclusion, 60 elderly patients with hypertension and depression were randomly divided into treatment group and control group. The patients in both groups were treated with conventional western medicine for lowering blood pressure. The treatment group was treated with Bushen Jieyu decoction, once a day, once a half hour after morning and evening meal, the course of treatment was 4 weeks. To observe the total curative effect of the two groups after treatment, the changes of TCM symptom score, TCM disease grading, blood pressure, Hamilton Depression scale (Hamd) score, elderly Depression scale (GDS) score, and plasma BDNF substance P level before and after treatment. And do statistical processing. Results: the total effective rate of TCM syndromes in the treatment group was 93.33%, which was significantly different from that in the control group (73.33%) (P0.05); the classification of TCM disease in the treatment group was obviously improved (P0.05), and was better than that in the control group; the systolic and diastolic blood pressure in the treatment group decreased to the normal range. The treatment group was superior to the control group (P0.05), the total effective rate of lowering blood pressure in the treatment group was 93.33, the control group was 83.4, the treatment group had significant difference (P0.05), the Hamd and GDS scores of the treatment group were significantly decreased (P0.01), and better than that of the control group. After treatment, the plasma BDNF in the treatment group was significantly higher than that before treatment (P0.05) and substance P was significantly lower than that before treatment (P0.05), and there was a significant difference between the treatment group and the control group. Conclusion: Bushen Jieyu decoction can improve the curative effect, improve the symptoms of TCM, reduce the grading of TCM condition, improve the blood pressure level, reduce the score of Hamd GDS scale and regulate the level of BDNFP in elderly patients with hypertension and depression. The treatment of elderly patients with hypertension with depression is effective and worthy of further clinical promotion and application.
