穴位埋线治疗脾胃虚弱型NERD患者红外皮温变化与反流症状的观察研究

【摘要】：目的:通过红外皮温热像仪监测"腹五穴"埋线治疗前后非糜烂性反流病患者督脉穴位红外皮温的变化以及反流症状的改善情况,讨论红外热像仪能否作为非糜烂性反流病的检测手段及"腹五穴"埋线治疗非糜烂性反流病的有效性及相关机理研究。方法:将纳入本研究的60例患者按照随机方分为对照组和治疗组。对照组给予枸橼酸莫沙必利分散片5mg,口服,一天三次;联合雷贝拉唑肠溶片10mg口服,一天两次,4周为1疗程。治疗组予"腹五穴"埋线治疗,每周埋线1次,疗程4周。观察患者治疗前后反酸、烧心、胸痛、反食症状改善程度并结合RDQ问卷调查表进行症状分析;通过监测治疗前后督脉穴位(大椎、至阳、命门)红外皮温的变化情况来探讨埋线治疗非糜烂性反流病的机理。全部实验数据均采用SPSS 23.0分析软件进行统计分析,计量资料均采用均数±标准差(X±S)表示,正态分布数据采用t检验,非正态分布数据采用秩和检验;计数资料采用X2检验;等级资料采用秩和检验处理。结果:治疗组与对照组总有效率分别为96.6%和83.3%,两组比较,差异有非常显著性意义(P0.05)。通过红外热成像仪观察督脉上穴位,发现治疗组中大椎穴升高2.6度,至阳穴升高1.8度,命门升高2.2度,且具有统计学意义。结论:通过RDQ量表的症状积分说明"腹五穴"埋线治疗非糜烂性反流病具有良好的临床效果,疗效明显优于单纯西药治疗。并且在实验观察过程中,我们发现红外热成像监测能快速有效的监测出该病的症状特点,故我们大胆的提出可将红外热成像检测技术应用于非糜烂性反流病的的诊断中,作为其检测手段之一。
[Abstract]:Objective: to monitor the changes of infrared skin temperature and the improvement of symptoms of reflux in patients with non-erosive reflux disease before and after the treatment of "abdominal five points" embedding with infrared skin temperature thermograph. This paper discusses whether the infrared thermal imager can be used as a detection method of non-erosive reflux disease and the effectiveness and related mechanism of "abdominal five points" embedding thread in the treatment of non-erosive reflux disease. Methods: 60 patients were randomly divided into control group and treatment group. The control group was given 5 mg of mosapride citrate dispersible tablets, three times a day, and rabeprazole enteric-coated tablet 10mg was given orally twice a day for 4 weeks as a course of treatment. The treatment group was treated with "abdominal five points" catgut embedding once a week for 4 weeks. Observe the improvement of symptoms of regurgitation, heartburn, chest pain and food reaction before and after treatment, and analyze the symptoms with RDQ questionnaire, monitor the acupoints of du pulse (Dazhui, Zhiyang) before and after treatment. The change of infrared skin temperature was used to explore the mechanism of non-erosive reflux disease treated by embedding thread. All the experimental data were analyzed by SPSS 23.0 software, the measurement data were expressed by the mean 卤standard deviation (X 卤S), the normal distribution data by t test, the non-normal distribution data by rank sum test, the count data by X2 test. Rank data are processed by rank sum test. Results: the total effective rates of the treatment group and the control group were 96.6% and 83.3%, respectively. The difference between the two groups was significant (P0.05). Observation of the point on du Mai by infrared thermography showed that Dazhui acupoint increased 2.6 degrees to Yang point 1.8 degrees and Shengmen raised 2.2 degrees in the treatment group and had statistical significance. Conclusion: the symptom integral of RDQ scale shows that the treatment of non-erosive reflux disease with "abdominal five points" embedding thread has a good clinical effect, and the curative effect is obviously superior to that of western medicine alone. And in the course of experimental observation, we found that infrared thermal imaging monitoring can quickly and effectively detect the symptoms of the disease, so we boldly put forward the infrared thermal imaging detection technology can be used in the diagnosis of non-erosive reflux disease. As one of its detection methods.
