观察运脾益肾，降浊通络法治疗早中期糖尿病肾病的临床疗效

【摘要】：研究目的:观察运脾益肾,降浊通络法治疗早中期糖尿病肾病的临床疗效。研究方法:根据2型糖尿病、早中期糖尿病肾病的临床诊断标准,采用随机对照方法,纳入60例早中期糖尿病肾病患者,按1:1对照原则,随机分为中药组、西药组,在常规治疗的基础上,前者给予运脾益肾汤治疗,后者给予缬沙坦(80mg)治疗。12周为1疗程,1个疗程后,进行疗效评价。统计两组患者的症状积分、MALB、24h尿蛋白定量,血糖血脂,纤维蛋白原,肾功、GFR、UAER等,采用spss17.0进行统计学分析。研究结果:1.比较中药组与西药组减轻中医症状方面,前者总有效率是83%,后者总有效率是67%。2.运脾益肾汤有较好的降低尿蛋白作用,统计结果显示:P0.05。3.运脾益肾汤在控制血糖血脂方面,疗效突出,统计结果显示:P0.05。4.运脾益肾汤具有改善肾功能作用,统计结果显示:P0.05。5.运脾益肾汤在降血浆纤维蛋白原方面,疗效突出,统计结果显示:P0.05。结论:运脾益肾汤改善早中期糖尿病肾病患者的临床症状,降低尿蛋白、调节血糖血脂、改善肾功能、降低纤维蛋白原等方面,疗效突出。
[Abstract]:Objective: to observe the clinical effect of removing spleen and tonifying kidney and reducing turbidity and dredging collaterals in the treatment of early and middle stage diabetic nephropathy. Methods: according to the clinical diagnostic criteria of type 2 diabetes mellitus and early and middle stage diabetic nephropathy, 60 patients with early and middle stage diabetic nephropathy were randomly divided into Chinese medicine group and western medicine group according to the 1:1 control principle. On the basis of routine treatment, the former was treated with Yunpi Yishen decoction, while the latter was treated with valsartan (80mg) for 12 weeks as a course of treatment. After one course of treatment, the curative effect was evaluated. The symptom scores of the two groups were analyzed by spss17.0, including 24 hours urinary protein quantification, blood glucose, blood lipids, fibrinogen, renal function GFR UAER, etc. The result of the study was: 1. Compared with the traditional Chinese medicine group and the western medicine group, the total effective rate of the former is 833 and the latter is 67.2. Yunpi Yishen decoction has a better effect on reducing urine protein, and the statistical results show that: P0.05.3. Yunpi Yishen decoction in the control of blood glucose and lipids, the curative effect is outstanding, the statistical result shows that: P0.05.4. Yunpi Yishen decoction can improve renal function, and the statistical results show that: P0.05.5. The effect of Yunpi Yishen decoction on reducing plasma fibrinogen was remarkable, and the statistical results showed that: P 0. 05. Conclusion: Yunpi Yishen decoction can improve the clinical symptoms, reduce urinary protein, regulate blood glucose and blood lipid, improve renal function and reduce fibrinogen in patients with early and middle stage diabetic nephropathy.
