基于中医传承辅助系统治疗慢性糜烂性胃炎的用药规律分析

【摘要】：第一部分基于中医传承辅助系统当代医家治疗慢性糜烂性胃炎用药规律分析目的:基于中医传承辅助系统(V2.5)软件,探讨当代医家治疗慢性糜烂性胃炎的中医临床用药规律,以期更好的指导临床遣方用药,为进一步研制新药奠定基础。方法:基于中医传承辅助系统(V2.5)软件对2007-2016年公开发表在中国学术文献总库(CNKI)、维普、万方中文期刊数据库的治疗慢性糜烂性胃炎的中医处方进行数据挖掘,探讨当代医家治疗慢性糜烂性胃炎的组方用药特点及配伍应用规律。结果:通过对筛选出来的439首处方进行分析,结果显示,共使用219味中药,前5味高频次使用中药依次为:甘草、黄连、半夏、白术、白及;药物四气统计显示,以寒性和温性药物使用最多;药物五味统计显示,苦味药使用最多;药物归经分析显示出现频率较高的为脾、胃、肺经;选择合适的支持度个数(40)及置信度(0.9),利用关联规则分析,可导出7条用药规则;利用熵层次聚类法提炼组合出治疗新处方共13首。结论:1当代医家治疗慢性糜烂性胃炎多以健脾祛湿为主,寒温并用,攻补兼施,注重使用脾、胃、肺经中的药物,用药广泛。2利用中医传承辅助系统(V2.5)软件完成了对处方的录入、管理及分析,对于研究中药组方规律、发现新处方有重要意义,对临床治疗慢性糜烂性胃炎具有一定的指导作用。第二部分基于中医传承辅助系统王彦刚教授治疗慢性糜烂性胃炎用药规律分析目的:基于中医传承辅助系统(V2.5)软件,探讨王彦刚教授治疗慢性糜烂性胃炎的中医临床用药规律,为中医药治疗慢性糜烂性胃炎提供一个客观的、系统的统计归类及分析计算方法,为临床治疗慢性糜烂性胃炎提供新思路及参考。方法:基于中医传承辅助系统(V2.5)软件对2013-2016年王彦刚教授在河北省中医院门诊治疗慢性糜烂性胃炎患者的首诊中医处方进行数据挖掘,探讨王彦刚教授治疗慢性糜烂性胃炎用药规律及经验,最后挖掘新处方。结果:通过对508例患者的基本情况及首诊处方进行分析,结果显示,508例患者中,男性247例,女性261例;年龄在51-60岁的人数最多;共使用220味中药,前5味高频次使用中药依次为:黄芩、半夏、黄连、竹茹、茵陈;药物四气统计显示,以寒性和温性药物使用最多;药物五味统计显示,苦味药使用最多;药物归经显示出现频率较高的为胃、脾、肝经;中医证型分类统计显示,浊毒内蕴证最多,其次为脾胃湿热证;将220味中药按功效分类,共使用18类中药,前5位使用药味数由高到低分别为清热药、补虚药、活血化瘀药、利水渗湿药、理气药;选择合适的支持度个数(400)及置信度(0.9),利用关联规则分析,可导出15条用药规则;利用熵层次聚类法提炼组合出治疗新处方共18首。结论:1王彦刚教授认为浊毒内蕴为慢性糜烂性胃炎的主要病机,用药多以化浊解毒为主,形成了通滞、化积、祛湿、涤痰、散瘀、解毒于“六位一体”的综合性治疗网络,强调治胃勿忘肝,用药集中。2利用中医传承辅助平台(V2.5)软件完成了对处方的录入、管理及分析,对于研究中药组方规律、发现新处方及辅助传承名老中医经验提供了有力工具,对临床治疗慢性糜烂性胃炎亦具有一定的指导意义。

Methods: Based on the software of traditional Chinese medicine V2.5 (V2.5), data mining on the first doctor's prescription of Professor Wang Yangang in the treatment of chronic erosive gastritis in the outpatient clinic of Hebei province was carried out, and the prescriptions of Professor Wang Yangang on the treatment of chronic erosive gastritis were discussed. Results: through the analysis of the basic situation and the first prescription of the 508 cases, the results showed that among the 508 patients, 247 cases were male, 261 cases were female, and the number of people aged 51-60 years was the most. Conclusion: 1 professor Wang Yangang believed turbid poison. It is the main pathogenesis of chronic erosive gastritis. The drug is mainly treated with turbid and detoxification. It forms the comprehensive treatment network of "six in one" with stagnation, accumulation, dispelling dampness, removing phlegm, dispersing stasis and detoxifying the "six in one", and concentrating on the administration and analysis of the prescription by using the traditional Chinese medicine V2.5 software. In the study of the rules of the prescription of Chinese medicine, we found the new prescription and the help to inherit the experience of the old Chinese medicine. It also has a certain guiding significance for the clinical treatment of chronic erosive gastritis.
