舒筋外洗颗粒在前交叉韧带重建术后早期康复中的应用研究

【摘要】：目的：通过观察舒筋外洗颗粒配合功能康复训练对前交叉韧带(Anterior Cruciate Ligament, ACL)重建术后膝关节功能康复的影响,探讨分析此制剂的临床疗效及其作用机理,为ACL重建术后膝关节功能的快速康复找到一种简便廉验的方法。方法：于2015年4月至2016年1月期间,从广州中医药大学第三附属医院选取符合纳入标准的60例ACL断裂患者。采用随机数字表法,将60例患者随机分为治疗组和对照组,每组各30例。治疗组在伤口愈合后接受为期4周的舒筋外洗颗粒熏洗疗法配合康复训练,而对照组仅给予相同的康复训练。观察记录两组患者在ACL重建术后2周、4周、6周及12周时患膝肿胀程度、VAS疼痛评分、膝关节活动度(ROM)、Lysholm膝关节评分,记录患者手术后重返工作岗位所需时间,收集数据并进行统计分析。结果：1、两组患者的一般资料及术后2周时患膝肿胀程度、VAS疼痛评分、ROM、Lysholm膝关节评分差异均无统计学意义(P0.05),治疗组和对照组具有可比性；2、术后4周和6周,两组患者之间的患膝肿胀程度、VAS疼痛评分、ROM、Lysholm膝关节评分比较差异均有统计学意义(P0.05),治疗组优于对照组；3、术后12周两组间各项观察指标经检验差异均无统计学意义(P0.05)；4、治疗组术后重返工作岗位所需时间少于对照组,差异有统计学意义(P0.05)；5、两组患者在整个研究过程中均无不良反应。结论：舒筋外洗颗粒熏洗疗法配合功能康复训练能减轻ACL重建术后的膝关节肿胀、疼痛,增加膝关节活动度,改善膝关节功能,进而缩短术后康复时间。此康复方案实用安全、疗效确切,值得在临床上推广。
[Abstract]:Objective: to observe the effect of Shujin Ouxi granule combined with functional rehabilitation training on the functional rehabilitation of knee joint after anterior cruciate ligament (ACL) (Anterior Cruciate Ligament, ACL) reconstruction, and to explore the clinical effect and mechanism of this preparation. To find a simple and inexpensive method for rapid rehabilitation of knee joint function after ACL reconstruction. Methods: from April 2015 to January 2016, 60 patients with ACL breakage were selected from the third affiliated Hospital of Guangzhou University of traditional Chinese Medicine. 60 patients were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group received 4 weeks of fumigation therapy combined with rehabilitation training after wound healing, while the control group only received the same rehabilitation training. To observe and record the degree of swelling of knee and the knee joint score of (ROM) and Lysholm after ACL reconstruction, and to record the time needed to return to work after operation, and to observe and record the degree of swelling of knee and the score of knee joint in two groups at 2 weeks, 4 weeks, 6 weeks and 12 weeks after ACL reconstruction. Collect data and make statistical analysis. Results there was no significant difference in general data and VAS pain score and Lysholm knee score between the two groups at 2 weeks after operation (P0.05). There was no significant difference between the treatment group and the control group at 4 and 6 weeks after operation. There were significant differences in VAS pain score and Lysholm knee score between the two groups (P0.05). The treatment group was better than the control group (P < 0.05). There was no significant difference in the observation indexes between the two groups 12 weeks after operation (P0.05). 4. The time needed to return to work after operation in the treatment group was less than that in the control group, the difference was statistically significant (P0.05). There were no adverse reactions in the whole course of the study between the two groups. Conclusion: the fumigation and washing therapy combined with functional rehabilitation training can reduce the swelling and pain of knee joint after ACL reconstruction, increase the range of knee motion, improve the function of knee joint, and then shorten the time of postoperative rehabilitation. This rehabilitation program is practical, safe and effective. It is worth popularizing in clinic.
