旋扳手法结合针刀松解剥离术治疗臀上皮神经卡压综合征的临床观察

【摘要】：目的:通过随机对照的方法,评价旋扳手法配合针刀松解剥离治疗臀上皮神经卡压综合征的临床疗效,探寻臀上皮神经卡压综合征的最重要原因,并对其进行靶向性的治疗。为寻求治疗本病的有效性治疗方法提供临床科学依据。方法:本研究的采纳的患者均为黑龙江中医药大学附属第一医院骨二科门诊、哈尔滨医科大学附属第四院疼痛科门诊,就诊时间为2015年01月至2016年03月,符合臀上皮神经卡压症状临床诊断的患者60例。将60例患者按照随机数表法随机分为旋扳手法结合针刀松解剥离组(治疗组)和常规针刀松解剥离(对照组)各30例。每周治疗一次,两次为一疗程。经过疗前治疗评分以及治疗后评分信息采集,评定标准依据JOA下腰痛评分以及VAS疼痛积分进行评定。结果:各组在年龄、性别、病程分布上进行比较,差别无统计学意义(P0.05)治疗前两组VAS评分和JOA评分比较无统计学意义,具有可比性(P0.05);治疗2个疗程后,在临床疗效上,两组之间的疗效的差异具有统计学意义(P0.05)。治疗前后每组的VAS评分和JOA评分比较均有变化,差异具有统计学意义(P0.05)而治疗组又优于对照组,差异具有统计学意义(P0.05)结论:小针刀松解剥离的方法疗效确切,安全可靠,可以改善患者的疼痛等临床症状,有效地提高病人的工作与生活质量。旋扳手法结合针刀松解剥离方法治疗疗效优于局部松解剥离法。患者易于接受,治疗彻底,经济、效果明显,此方法值得临床推广。
[Abstract]:Objective: To evaluate the clinical efficacy of manipulation of rotation and pulling combined with Acupotomy in the treatment of gluteal epithelial nerve entrapment syndrome, to explore the most important cause of gluteal epithelial nerve entrapment syndrome, and to carry out targeted treatment. The study included 60 patients who met the clinical diagnosis of gluteal nerve entrapment symptoms from January 2015 to March 2016. Sixty patients were randomly divided into wrenches according to the random number table method. Methods 30 cases in each group were treated once a week and twice as a course of treatment. After pre-treatment score and post-treatment score information collection, evaluation criteria were evaluated according to JOA low back pain score and VAS pain score. There was no significant difference in VAS score and JOA score between the two groups before and after treatment (P 0.05). After two courses of treatment, there was significant difference in clinical efficacy between the two groups (P 0.05). The difference was statistically significant (P 0.05). Conclusion: The small needle knife release peeling method is effective, safe and reliable, can improve the pain and other clinical symptoms of patients, effectively improve the quality of work and life of patients. Local release and stripping method is easy to be accepted by patients, thorough treatment, economical and effective. This method is worthy of clinical promotion.
