刺血疗法结合电针治疗偏头痛的临床研究

【摘要】:目的:观察刺血疗法结合电针治疗偏头痛的临床疗效,并与单纯电针治疗偏头痛和口服西药治疗偏头痛对照,分别比较三种治疗方法在首次治疗结束30分钟后的止痛效果及一个疗程治疗结束、结束后一个月、结束后三个月的治疗效果。方法:本研究对收集的105例符合要求的偏头痛患者,随机分为刺血结合电针治疗组(观察组)和单纯电针对照组(电针组),口服西药对照组(西药组),每组均35例。观察组采用点刺放血结合电针的方法进行治疗,单纯电针对照组采用单纯电针的方法进行治疗,口服西药对照组采用口服麦角胺咖啡因片和氟桂利嗪进行治疗,三组均进行12天的治疗,观察首次治疗结束30分钟后的视觉模拟评分(VAS评分)及一个疗程治疗结束、结束后一个月、结束后三个月的头痛积分,通过治疗前后的积分对比来观察刺血疗法结合电针治疗偏头痛的疗效,比较三种治疗方法的疗效差异。结果:1.首次治疗结束30分钟后:三组患者首次治疗结束30分钟后VAS评分显著下降,但观察组下降值优于电针组和西药组(p0.01)。2.一个疗程治疗结束:三组头痛积分显著下降,但观察组头痛积分下降值和总有效率优于电针组和西药组(p0.01)。3.治疗结束1个月后,三组头痛积分显著下降,但观察组头痛积分下降值(p0.05)和总有效率(p0.01)优于电针组和西药组。4.治疗结束3个月后,三组头痛积分显著下降,但观察组头痛积分下降值(p0.01)和总有效率(p0.05)优于电针组和西药组。5.观察组治疗后1个月、3个月疗效稳定,电针组治疗后1个月疗效稳定、3个月疗效下降,西药组治疗后1个月疗效稳定、3个月疗效稳定显著下降。6.治疗期间,观察组不良反应率与电针组相当(p0.05),低于西药组(p0.01)。结论:1.刺血结合电针治疗、单纯电针治疗、口服西药治疗三种治疗方法对偏头痛均有较好的即刻止痛效果,但刺血结合电针治疗偏头痛的即刻止痛效果明显优于单纯电针治疗和口服西药治疗。2.刺血结合电针治疗偏头痛在近期疗效、短期疗效、中长期疗效上都明显优于单纯电针治疗和口服西药治疗,而且疗效持续稳定。3.刺血结合电针治疗偏头痛疗效确切,安全性高,值得推广。
[Abstract]:Objective: to observe the clinical effect of blood pricking therapy combined with electroacupuncture on migraine, and to compare it with the treatment of migraine by electroacupuncture alone and by oral western medicine. The analgesic effects of the three treatments were compared 30 minutes after the first treatment and one month after the end of a course of treatment and three months after the end of the treatment. Methods: 105 patients with migraine were randomly divided into treatment group (observation group) and electroacupuncture control group (electroacupuncture group) and oral western medicine control group (western medicine group) 35 cases in each group. The observation group was treated by puncturing and bleeding combined with electroacupuncture, the control group was treated by electroacupuncture alone, and the control group by oral administration of ergohydrazine caffeine tablets and flunarizine. The visual analogue score (VAS score) 30 minutes after the first treatment and the headache score of one course of treatment, one month after treatment and three months after the end of the first treatment were observed in all the three groups. The curative effect of blood pricking therapy combined with electroacupuncture on migraine was observed by the integral comparison before and after treatment. The result is 1: 1. 30 minutes after the first treatment, the VAS score of the three groups decreased significantly after the first treatment, but the decreasing value of the observation group was better than that of the electroacupuncture group and the western medicine group (p0.01) .2. At the end of a course of treatment, the headache score of the three groups decreased significantly, but the decreasing value and the total effective rate of the headache score in the observation group were better than those in the electroacupuncture group and the western medicine group (p0.01) .3. One month after the treatment, the headache score of the three groups decreased significantly, but the decreasing value of headache score (p0.05) and the total effective rate (p0.01) in the observation group were better than those in the electroacupuncture group and the western medicine group. After 3 months of treatment, the headache score of the three groups decreased significantly, but the decreasing value of headache score (p0.01) and the total effective rate (p0.05) in the observation group were better than those in the electroacupuncture group and the western medicine group. In the observation group, the curative effect was stable at 1 month and 3 months after treatment. In the electroacupuncture group, the curative effect was stable at 1 month and decreased in 3 months. In the western medicine group, the curative effect was stable at 1 month and decreased significantly at 3 months. During the treatment, the adverse reaction rate in the observation group was similar to that in the electroacupuncture group (p 0.05), and was lower than that in the western medicine group (p 0.01). Conclusion 1. Blood pricking combined with electroacupuncture, simple electroacupuncture and oral western medicine have good immediate analgesic effect on migraine. But the immediate analgesic effect of blood pricking combined with electroacupuncture on migraine was better than that of electroacupuncture alone and oral western medicine. Blood pricking combined with electroacupuncture in the treatment of migraine in the short-term, medium- and long-term effects are significantly better than simple electroacupuncture treatment and oral western medicine treatment, and the efficacy is stable. 3. Blood pricking combined with electroacupuncture in the treatment of migraine is effective, safe and worth popularizing.
