活血止痛膏治疗膝痹病瘀血痹阻证的临床研究

【摘要】：目的：目前探究对于膝骨性关节炎的治疗方法是重要的一项临床课题。本课题通过对活血止痛膏治疗膝骨性关节炎的临床疗效的观察以及其对于人体膝关节液中相关因子指标的影响,提供了中医药更安全有效的应用到膝骨性关节炎的治疗依据。方法：本研究选择44例膝骨性关节炎患者,入院后取所有确诊患者膝关节液,用于测定TNF-α、IL-6含量,运用ELISA法来检查TNF-α和IL-6含量,选用上海江莱生物科技有限公司试剂盒。操作均按操作试剂盒说明书来进行。同时予以VAS评分及麦克马斯特大学骨关节炎指数(WOMAC)评分。然后膝关节外敷活血止痛膏,每次4小时,每天1次,治疗2周后和治疗4周后,用同样方法复查TNF-α、IL-6及复测VAS评分及WOMAC评分。通过观察VAS评分、WOMAC评分和血清TNF-α、IL-6数值的变化,从临床疗效上探究活血止痛膏对于膝骨性关节炎的效果。结果：1.安全性方面：一位患者因皮肤过敏退出本次临床试验观察,其余患者均未出现不良反应。2.视觉疼痛评分(VAS)方面：经外敷活血止痛膏,42位患者疼痛症状较治疗前有改善,用药2周、用药4周与治疗前比较均存在差异(P0.05)有统计学意义。3.骨关节炎指数(WOMAC)方面：经外敷活血止痛膏,42位患者WOMAC指数较治疗前有降低,用药2周、用药4周与治疗前比较均存在差异(P0.05)有统计学意义。4.生化指标方面：经外敷活血止痛膏,42位患者血清中TNF-α及IL-6数值较治疗前有所减低,用药2周、用药4周与治疗前比较均存在差异(P0.05)有统计学意义。结论：活血血-J_止痛膏对于减轻膝骨性关节炎患者疼痛等临床症状是有效的,其可以使膝关节功能改善,降低血清TNF-a及IL-6含量,不良反应较低,副作用较小,对于治疗膝骨性关节炎是安全可行的。
[Abstract]:Objective: to explore the treatment of knee osteoarthritis is an important clinical topic. By observing the clinical curative effect of Huoxue Zhitong ointment in treating knee osteoarthritis and its influence on the related factors in human knee joint fluid, this subject provides a more safe and effective basis for the treatment of knee osteoarthritis with traditional Chinese medicine. Methods: in this study, 44 patients with knee osteoarthritis were selected. The knee joint fluid was taken from all the patients after admission, and the contents of TNF- 伪 and IL-6 were measured by ELISA method. The kit of Shanghai Jianglai Biotechnology Co., Ltd was used to detect the content of TNF- 伪 and IL-6. The operation is carried out according to the instructions of the kit. At the same time, the VAS score and the (WOMAC) score of the McMaster University osteoarthritis index were given. Then the knee joint was applied with Huoxue Zhitong ointment, 4 hours each time, once a day, 2 weeks after treatment and 4 weeks after treatment, TNF- 伪 IL-6, VAS score and WOMAC score were re-examined with the same method. The effect of Huoxue Zhitong ointment on knee osteoarthritis was investigated by observing the changes of VAS score and serum TNF- 伪 and IL-6 in order to find out the clinical efficacy of Huoxue Zhitong ointment. The result is 1: 1. Safety: one patient withdrew from the clinical trial because of skin allergy, and none of the other patients had adverse effects. 2. 2. Visual pain score (VAS): pain symptoms of 42 patients were improved after external application of Huoxue Zhitong ointment. There were significant differences in pain symptoms between 2 weeks and 4 weeks after treatment (P0.05). Osteoarthritis index (WOMAC): the WOMAC index of 42 patients treated with external application of Huoxue Zhitong ointment was lower than that before treatment. There were significant differences in WOMAC index between 2 weeks and 4 weeks after treatment (P0.05). Biochemical indexes: the values of TNF- 伪 and IL-6 in serum of 42 patients with Huoxue Zhitong ointment were lower than those before treatment. There were significant differences in the values of TNF- 伪 and IL-6 between 2 weeks and 4 weeks after treatment (P0.05). Conclusion: Huoxue Xue-J _ Zhitong ointment is effective in relieving pain and other clinical symptoms in patients with knee osteoarthritis. It can improve knee joint function, reduce serum TNF-a and IL-6 levels, lower adverse reactions and less side effects. It is safe and feasible for the treatment of knee osteoarthritis.
