逍遥逐瘀汤对气滞血瘀型盆腔炎性疾病后遗症的临床效果观察

【摘要】：目的:评价逍遥逐瘀汤治疗气滞血瘀型盆腔炎性疾病后遗症的临床疗效,为其临床应用提供科学的依据。方法:采用随机对照的试验研究方法,共纳入2014年4月至2015年4月于河南中医药大学第一附属医院妇科门诊就诊的气滞血瘀型盆腔炎性疾病后遗症患者72例,分为两组,治疗组36例,给予逍遥逐瘀汤;对照组36例,给予红金消结胶囊,两组均于月经干净后3天开始用药,15天为一个疗程,经期停药,连用3个疗程。通过观察两组治疗前后临床症状、体征、疼痛评分、超声的变化情况来评价临床疗效。结果:(1)逍遥逐瘀汤在治疗气滞血瘀型盆腔炎性疾病后遗症的中医证候、局部体征、综合疗效的有效率分别为:97.22%、94.44%、97.22%,而对照组分别为:88.89%、83.33%、88.89%,经统计学分析,治疗组明显优于对照组(P0.05)。(2)两组在改善患者疼痛疗效方面,治疗组有效率为97.22%,对照组为80.56%,差异有统计学意义(P0.05)。(3)通过对患者盆腔积液及包块情况改变的观察发现,治疗组疗后较疗前差异均有统计学意义(P0.05),对照组在盆腔包块方面疗后较疗前差异有统计学意义(P0.05),但在盆腔积液上差异无统计学意义(P0.05),疗后组间比较差异有统计学意义(P0.05)。结论:(1)逍遥逐瘀汤能明显改善气滞血瘀型盆腔炎性疾病后遗症患者的症状、体征、疼痛程度,且效果优于红金消结胶囊组;(2)红金消结胶囊能缩小盆腔炎性包块,但对盆腔积液效果不明显;(3)逍遥逐瘀汤可促进盆腔积液及包块的吸收,且治疗效果优于红金消结胶囊组。
[Abstract]:Objective: to evaluate the clinical efficacy of Xiaoyao Zhuyu decoction in treating the sequelae of pelvic inflammatory disease of qi stagnation and blood stasis type, and to provide scientific basis for its clinical application. Methods: from April 2014 to April 2015, 72 patients with the sequelae of Qi stagnation and Blood stasis pelvic inflammatory Disease were enrolled in the gynecological outpatient clinic of the first affiliated Hospital of Henan University of traditional Chinese Medicine, and were divided into two groups. 36 cases in treatment group were given Xiaoyao Zhuyu decoction and 36 cases in control group were given Hongjin Xiaojie capsule. The clinical effects were evaluated by observing the changes of clinical symptoms, signs, pain scores and ultrasound before and after treatment. Results: (1) the effective rate of Xiaoyao Zhuyu decoction in treating the sequelae of qi stagnation and blood stasis type pelvic inflammatory disease in traditional Chinese medicine, local signs and comprehensive curative effect were respectively: 1. 97.222.44 and 94.447.222.22, respectively, while in the control group it was 83.338.89. The effective rate of treatment group was 97.22 and that of control group was 80.56. The difference was statistically significant (P0.05). (3) through the observation of the changes of pelvic effusion and mass in patients, the effective rate of treatment group was 97.22 and the control group was 80.56.The difference was significant (P0.05). (3). There were significant differences after treatment in the treatment group compared with before treatment (P0.05). In the control group, there was significant difference in the pelvic mass after treatment (P0.05), but there was no significant difference in the pelvic effusion (P0.05), the difference between the treatment group and the control group was statistically significant (P0.05). Conclusion: (1) Xiaoyao Zhuyu decoction can obviously improve the symptoms, signs, pain degree and the effect of Hongjin Xiaojie capsule group, and (2) Hongjinxiaojie capsule can reduce the pelvic inflammatory mass. The effect of Xiaoyao Zhuyu decoction on pelvic effusion was not obvious. (3) Xiaoyao Zhuyu decoction could promote the absorption of pelvic effusion and mass, and the therapeutic effect was better than that of Hongjin Xiaojie capsule group.
