郭兆刚教授治疗儿童腺样体肥大的临证学术思想总结

【摘要】：目的:儿童腺样体肥大作为儿童常见病,对儿童生长发育所产生的影响,得到了普遍重视。郭兆刚教授是云南省名中医,擅长治疗耳鼻喉科各种疾病。本文通过临床归纳郭兆刚教授对小儿腺样体肥大的临证治法、临床用药、及加减规律,以系统总结郭兆刚教授对小儿腺样体肥大的临床思想及学术经验使其对本病的证治思想得到继承与发扬,为中医临床提供借鉴与参考。方法:阅读相关古代文献,检索现代医学对儿童腺样体肥大的相关文献论述,同时在跟师的学习中,收集郭兆刚教授治疗本病的病例资料、诊疗信息,并做好门诊记录及笔记。进而归纳总结郭兆刚教授对本病的病机认识、治疗方法、用药规律。结果:郭兆刚教授认为小儿脏腑娇嫩,形气未充是儿童腺样体肥大发病的内在因素,在此基础上外邪内犯,内外合邪,壅阻气机,化热、生痰、致瘀,变生腺样体肥大诸证。临床当通过“望、闻、问、切、查”五诊,仔细辨之,根据辨证予以益气固表、疏风散邪、宣肺通窍、益气活血、行气通窍等治法,达到整体论治的观念。从而使儿童达到阴平阳秘的状态,打断因腺样体肥大引起的恶性循环,恢复其正常良性循环,待腺样体自然萎缩,则其引发的诸证消失。避免了激素及手术治疗可能带来的危害,取得了较好的临床效果。结论:通过本次的研究总结了郭兆刚教授对儿童腺样体肥大病因病机、临床辩证及用药规律。导师以整体观进行论治,审证求因,从因论治。重视脏腑娇嫩的本因,辅以通窍的思想,临床治疗中效果显著,缓解了腺样体肥大儿童的痛苦,发挥了中药治疗的安全性、持续性,显效性等特点,对中医临床医生治疗本病给予了参考与指导。
[Abstract]:Objective: the effect of adenoid hypertrophy on children's growth and development as a common disease in children has been paid more and more attention. Professor Guo Zhaogang is a famous traditional Chinese medicine in Yunnan Province, specializing in the treatment of various diseases of otolaryngology. In this paper, Professor Guo Zhaogang's clinical treatment of adenoid hypertrophy in children, clinical medication, and the rule of addition and subtraction are summarized. By systematically summarizing Professor Guo Zhaogang's clinical thought and academic experience on adenoid hypertrophy in children, his thought on syndrome and treatment of this disease has been inherited and carried forward, which provides reference and reference for the clinical practice of traditional Chinese medicine. Methods: to read the relevant ancient literature, to search the literature about children's adenoid hypertrophy in modern medicine, at the same time to collect the case data, diagnosis and treatment information of Professor Guo Zhaogang in the study of follow teacher, and to make good records and notes of outpatient service. Then summarize Professor Guo Zhaogang's understanding of the pathogenesis of the disease, treatment methods, drug use law. Results: professor Guo Zhaogang thought that the internal factors of children's adenoid hypertrophy were delicate viscera and unfilled form qi. On the basis of this, internal and external pathogenic factors, internal and external evil, obstruction of qi, heat, phlegm, blood stasis and hypertrophy of adenoid were all syndromes. Clinical practice should pass "look, smell, ask, cut, check" five diagnoses, distinguish it carefully, according to differentiation to give Yiqi solid table, dispersing wind and dispersing evil, purging lung and opening orifices, benefiting qi and activating blood, and so on, so as to achieve the concept of holistic treatment. In order to make the children reach the state of yin and pacification, break the vicious circle caused by adenoid hypertrophy, restore its normal virtuous circle, and when the adenoid atrophies naturally, the symptoms will disappear. It avoids the possible harm of hormone and surgical treatment, and obtains better clinical effect. Conclusion: professor Guo Zhaogang has summarized the etiology, pathogenesis, clinical dialectical and drug use of adenoid hypertrophy in children. Mentors to the overall view of treatment, examination of the cause of evidence, from the cause of treatment. Paying attention to the delicate cause of the viscera, accompanied by the idea of opening the orifices, has had a remarkable effect in clinical treatment, alleviated the suffering of children with adenoid hypertrophy, and brought into play the characteristics of the safety, sustainability, and effectiveness of the treatment of traditional Chinese medicine. It provides reference and guidance for the treatment of this disease by TCM clinicians.
