超微针刀配合中药内服治疗跟痛症的临床观察

【摘要】：目的:通过整理、分析相关观察指标,并通过与局部封闭治疗相比较,回顾分析超微针刀配合中药内服治疗跟痛症的临床疗效,以便为跟痛症治疗方案的选择提供理论和临床依据。方法:选取52例于2014年06月—2015年12月就诊于武汉市中医医院骨伤科门诊的符合该临床观察的纳入标准且经排除标准筛查的跟痛症病例进行回顾性分析。其中,26例(29跟)来自于被予以超微针刀配合中药内服治疗的病例,另外26例(29跟)来自于被予以局部封闭治疗的病例。由同一位医师记录治疗前、治疗后1周、治疗后2个月的疼痛视觉模拟评分(即VAS评分),并分别于治疗后1周、治疗后2个月进行疗效评定。所得资料及数据采用SPSS19.0统计软件进行处理、分析。结果:(1)VAS评分:治疗前与治疗后1周的两组病例组内VAS评分比较均为P0.05,有显著差异,组间VAS评分比较P0.05,无显著差异。治疗前与治疗后2个月两组病例组内VAS评分比较均有P0.05,有显著差异,治疗后2个月两组病例间的组间VAS评分比较P0.05,有显著差异。(2)疗效评定:治疗后1周两组总有效率均为96.15%,P0.05,无显著差异;治疗后2个月两组总有效率分别为96.15%和80.77%,P0.05,有显著差异。结论:通过该临床观察,我们发现在治疗后1周治疗组和对照组的VAS评分均有下降且两组组间比较无显著差异,说明从短期疗效看两者均能缓解足跟部疼痛,且治疗组和对照组疗效相当;在治疗后2个月治疗组的VAS评分继续下降且与治疗前相比有显著差异,而对照组的VAS评分较治疗后1周上升、但与治疗前有显著差异,说明从长期疗效看超微针刀配合中药内服能明显缓解足跟部疼痛,而局部封闭治疗的疗效欠理想。治疗后1周两组有效率无显著差异,说明超微针刀配合中药内服治疗和局部封闭治疗均能缓解足跟部疼痛,且两者疗效相当;治疗后2个月,两组有效率有显著差异,治疗组的总有效率较对照组高,说明超微针刀配合中药内服治疗的疗效较局部封闭更理想。所以,对于跟痛症的治疗,超微针刀配合中药内服治疗和局部封闭治疗均能取得较理想的短期疗效,而从远期疗效来看,超微针刀配合中药内服治疗更具优势。其中,超微针刀主要发挥局部治疗作用,而内服中药能够调理机体阴阳,进而恢复“阴平阳秘”的生理状态,从而使跟痛症得到根本上的治疗。超微针刀配合中药内服治疗跟痛症的疗效确切,值得推广。
[Abstract]:Objective: to analyze the clinical efficacy of ultrafine needle knife combined with traditional Chinese medicine in the treatment of calcaneal pain by sorting out the relevant observation indexes and comparing with local blocking therapy. In order to provide theoretical and clinical basis for the treatment of heel pain. Methods: 52 cases of heel pain were selected from June 2014 to December 2015 in Department of Orthopaedics and Trauma, Wuhan traditional Chinese Medicine Hospital. Among them, 26 cases (29 heels) came from patients who were treated with ultrafine needle knife combined with traditional Chinese medicine, and 26 cases (29 heels) were treated with local blocking therapy. The pain visual analogue score (VAS score) was recorded by the same physician before treatment, 1 week after treatment and 2 months after treatment, and the curative effect was evaluated at 1 week after treatment and 2 months after treatment. The obtained data and data were processed and analyzed by SPSS19.0 statistical software. Results: (1) VAS score: there was significant difference in VAS score between the two groups before treatment and one week after treatment, but there was no significant difference in VAS score between the two groups (P 0.05). There were significant differences in VAS scores between the two groups before and after treatment (P 0.05). There was significant difference in VAS score between two groups 2 months after treatment (P0.05). (2) Evaluation of curative effect: the total effective rate of both groups was 96.15 and P0.05 at one week after treatment, and the total effective rate of two groups was 96.15% and 80.77% respectively after 2 months of treatment, with significant difference. Conclusion: through the clinical observation, we found that the VAS scores of the treatment group and the control group decreased 1 week after treatment, and there was no significant difference between the two groups, which indicated that both groups could relieve the heel pain in the short term. After 2 months of treatment, the VAS score of the treatment group continued to decrease and there was a significant difference compared with that before treatment, while the VAS score of the control group was higher than that of the control group at 1 week after treatment, but there was a significant difference between the two groups before and after treatment. The results showed that ultrafine needle knife combined with traditional Chinese medicine could obviously relieve the pain in the heel, but the curative effect of local sealing was not ideal. There was no significant difference in the effective rate between the two groups at 1 week after treatment, which indicated that the two groups could relieve the pain in the heel with ultrafine needle knife combined with traditional Chinese medicine and local blocking treatment, and the two groups had significant difference in the effective rate 2 months after treatment. The total effective rate of the treatment group was higher than that of the control group. Therefore, for the treatment of heel pain, ultrafine needle knife combined with traditional Chinese medicine and local blocking treatment can achieve an ideal short-term effect, but from the long-term effect, ultramicro needle knife combined with traditional Chinese medicine internal administration therapy has more advantages. Among them, the ultramicro needle knife mainly plays a local role, and taking Chinese herbal medicine can regulate the body yin and yang, and then restore the physiological state of "yin and soothe yang constipation", so that the pain with heel can be fundamentally treated. The curative effect of ultra-micro needle knife combined with traditional Chinese medicine in treating heel pain is definite and worth popularizing.
