解郁化瘀汤治疗中风后抑郁症的临床疗效观察

【摘要】：目的:通过对解郁化瘀汤治疗中风后抑郁症的临床疗效观察,探讨解郁化瘀汤的临床疗效及安全性,为中医药治疗中风后抑郁症提供客观的临床依据。方法:本研究将60例中风后抑郁症患者随机分为两组,治疗组30例,对照组30例。对照组给予盐酸氟西汀胶囊治疗;治疗组给予解郁化瘀汤治疗。在治疗前后,对两组患者进行汉密尔顿抑郁量表、神经功能缺损评分量表、日常生活能力量表及中医症候评分,比较两组的临床疗效,并评估治疗组的安全性。结果:(1)两组临床疗效:治疗组的总有效率为83.33%;对照组的总有效率为70%。两组总有效率比较,有统计学意义(P0.05),说明:治疗组疗效好于对照组。(2)治疗后两组间的汉密尔顿抑郁量表、日常生活能力量表都有所改善,但两组间比较无统计学意义(P0.05),说明两组都能改善患者的抑郁症状及日常生活能力,且两组疗效相当。(3)治疗后两组神经功能缺损量表评分及中医症候积分都有下降,两组间比较,差异有统计学意义(P0.05)。说明:两组治疗都能改善患者的神经功能缺损及中医症候,且治疗组好于对照组。(4)不良反应及安全性分析:在研究的过程中,治疗组未发生不良反应,安全性观测指标没有出现异常。说明解郁化瘀汤治疗中风后抑郁症安全性较高,无明显副作用。结论:解郁化瘀汤治疗中风后抑郁症有明确的疗效,且无明显副作用,有较好的临床运用价值。
[Abstract]:Objective: to investigate the clinical efficacy and safety of Jieyu Huayu decoction in the treatment of post-stroke depression and to provide an objective clinical basis for the treatment of post-stroke depression with traditional Chinese medicine. Methods: 60 patients with post-stroke depression were randomly divided into two groups: treatment group (n = 30) and control group (n = 30). The control group was treated with fluoxetine hydrochloride capsule and the treatment group with Jieyu Huayu decoction. Before and after treatment, the patients in the two groups were assessed with Hamilton Depression scale, Neurologic deficit scale, Daily living scale and TCM symptom scale. The clinical efficacy of the two groups was compared and the safety of the treatment group was evaluated. Results: (1) the total effective rate was 83.33 in the treatment group and 70 in the control group. The total effective rate of the two groups was significantly higher than that of the control group (P0.05). (2) after treatment, the Hamilton Depression scale and the activity of Daily living scale were improved. However, there was no significant difference between the two groups (P0.05), indicating that both groups could improve the depression symptoms and daily living ability of the patients, and the two groups had the same curative effect. (3) after treatment, the scores of the neurological deficit scale and the TCM symptom score of the two groups were decreased, and the comparison between the two groups was made. The difference was statistically significant (P0.05). Conclusion: both groups can improve the neurological function defect and TCM symptoms of the patients, and the treatment group is better than the control group. (4) adverse reactions and safety analysis: in the course of the study, there were no adverse reactions in the treatment group. There is no anomaly in the safety observation index. It shows that Jieyu Huayu decoction is safe in treating post-stroke depression and has no obvious side effects. Conclusion: Jieyu Huayu decoction has definite curative effect, no obvious side effect and good clinical application value.
