基于“扶阳学说”探讨“寒热并用”辨治糖尿病肾病

【摘要】："消渴肾病"的病机十分复杂,阴虚燥热为主,兼有瘀浊内阻贯穿病程始终,阳虚亦是其发生发展中的重要环节。而扶阳学派对"消渴"的认识以阳虚为本,认为消渴肾病亦有"坎中一阳"之不足,主张在益气养阴的基础治则上,注重解毒扶阳的运用,调节阴阳平衡。文章则基于"扶阳学说"的"扶阳"理念,由《伤寒论》"寒热并用"的治法得以启发,认为疾病治疗的关键在于辨证论治,有是证则用是药,鉴于"消渴肾病"的复杂病机,单纯"清热"或是"温补"不足以兼顾变生之病证,宜因时、因地、因人施治,治法上应予重视"寒热并用"。
[Abstract]:The pathogenesis of "Xiaoke Nephropathy" is very complicated, Yin deficiency dryness and heat is the main part, and the internal obstruction of stasis and turbidity runs through the course of the disease. Yang deficiency is also an important link in its occurrence and development. On the other hand, Fuyang school's understanding of "eliminating thirst" is based on the deficiency of Yang deficiency, which holds that there is also the deficiency of "clearing away thirst nephropathy", and advocates that in the basic treatment principle of invigorating qi and nourishing yin, the application of detoxification and nourishing yang should be paid attention to and the balance of yin and yang should be adjusted. Based on the theory of "Fuyang", the article is inspired by the treatment method of "combination of cold and heat". It is believed that the key of disease treatment lies in syndrome differentiation and treatment, and the syndrome is used as medicine. In view of the complex pathogenesis of "dispelling thirst nephropathy", the article points out that the treatment of disease is based on syndrome differentiation and treatment. Simply "clearing away heat" or "warming and nourishing" is not enough to give consideration to the syndromes of the disease which has changed into a new generation, and it is appropriate to pay attention to the combination of cold and heat when the cause is due to the cause, because of the land, because of the treatment of the people.
