中医综合疗法对慢性充血性心力衰竭伴抑郁治疗的疗效分析

【摘要】：目的:分析评价中医综合疗法治疗慢性充血性心力衰竭(CHF)伴抑郁的临床疗效。方法:将我院收治的150例慢性心力衰竭患者分为2组,其中对照组75例,治疗组75例。对照组患者给予单纯西医治疗,治疗组患者在西医治疗CHF的基础上加用自拟组方"疏肝理气解郁安神方"配合针灸、穴位按摩及耳穴贴压,并辅以中医情志护理,观察2组患者临床疗效、焦虑自评表(SAS)和抑郁自评表(SDS)得分、左心室射血分数、血浆BNP浓度、生活质量、不良反应及安全性指标,评价中医综合疗法的临床效果。结果:中医综合治疗CHF伴抑郁后患者总有效率为94.67%,显著高于单纯使用西药组(81.33%),治疗组SAS、SDS得分、LVEF、血浆BNP浓度、明尼苏达生活质量品评分改善程度均显著优于对照组(P0.05),不良反应发生情况得到明显改善。2组患者安全性指标差异无显著性。结论:在传统西医治疗CHF的基础上,联合中医综合疗法治疗抑郁,可以有效改善患者症状优于单纯应用西药治疗。中医药具有价廉质优、副作用少等优点,可以在缓解CHF患者症状的同时,改善患者抑郁状态,提高生活质量,长期优势明显。
[Abstract]:Objective: to evaluate the clinical efficacy of TCM combined therapy in the treatment of (CHF) with depression in chronic congestive heart failure (CHF). Methods: 150 patients with chronic heart failure were divided into two groups: the control group (n = 75) and the treatment group (n = 75). The patients in the control group were treated with western medicine alone, and the patients in the treatment group were treated with self-formulated prescription "soothing liver, regulating qi and relieving depression and soothing the spirit" combined with acupuncture and moxibustion, acupoint massage and auricular acupoint pressing, supplemented with Chinese medicine emotional nursing, on the basis of western medicine treatment. The clinical efficacy, (SAS) score, left ventricular ejection fraction (LVEF), plasma BNP concentration, quality of life (QOL), adverse reaction (ADR) and safety index were observed in the two groups. Results: the total effective rate of comprehensive treatment of CHF with depression was 94.677.It was significantly higher than that of western medicine alone (81.33%). The SAS,SDS score and plasma BNP concentration of LVEF, in the treatment group were significantly higher than those in the control group. The improvement degree of Minnesota quality of life score was significantly better than that of the control group (P0.05), and the occurrence of adverse reactions was significantly improved. There was no significant difference in safety indexes between the two groups. Conclusion: on the basis of the traditional western medicine treatment of CHF, the combination of traditional Chinese medicine and comprehensive therapy can effectively improve the symptoms of patients than that of western medicine alone. Traditional Chinese medicine has the advantages of low cost and less side effects. It can alleviate the symptoms of CHF patients, improve the depression state of patients, improve the quality of life, and have obvious long-term advantages.
