益肾蠲痹法治疗痛风漏诊脊柱关节病案例分析

【摘要】：脊柱关节病是一组有一些共同特定临床特点并与HLA-B27等位基因相关的疾病,是目前临床发病率高、致残率高的一大类风湿疾病,其临床表现多样,症状轻重不一,尤其是以外周关节炎起病的不典型患者临床常易误诊、漏诊。故报道1例因痛风病史而漏诊未分化脊柱关节炎的病例,经益肾蠲痹法联合泄浊化瘀法治疗后病情得到有效控制。结合临床资料及文献复习探讨本案漏诊原因,并总结朱婉华教授对脊柱关节病的认识、早期诊断的线索及临床治疗经验。
[Abstract]:Spondyloarthropathy is a group of diseases which have some common clinical characteristics and are associated with HLA-B27 alleles. It is a major rheumatoid disease with high clinical incidence and high disability rate. Its clinical manifestations are diverse and the severity of its symptoms varies. In particular, atypical patients with peripheral arthritis are often misdiagnosed and missed. This paper reports a case of undifferentiated spinal arthritis due to the history of gout. The disease was effectively controlled after treatment with Yishen Juanbi combined with removing turbidity and removing blood stasis. Combined with clinical data and literature review, the causes of missed diagnosis in this case were discussed, and professor Zhu Wanhua's understanding of spondyloarthropathy, early diagnosis clues and clinical treatment experience were summarized.
