芪楼固肾方对膜性肾病大鼠Nephrin及Podocin表达的影响

【摘要】:目的:本实验通过研究观察芪楼固肾方对膜性肾病大鼠尿蛋白、血清生化指标、肾脏病理以及肾组织Nephrin mRNA、Podocin mRNA表达的影响,探讨芪楼固肾方治疗膜性肾病的作用机制。方法:选取50只雄性Wistar大鼠,随机取10只作为空白组,其余大鼠预免疫一周后,隔日一次尾静脉注射牛血清白蛋白(C-BSA),制备膜性肾病大鼠模型。模型制备成功后将剩余40只大鼠随机分为4组,每组10只,即模型组、芪楼固肾方低剂量组、芪楼固肾方高剂量组、雷公藤多甙片组。动态观察56天,检测24小时尿蛋白定量(UTP)、血清总蛋白(TP)白蛋白(ALB)、肝功能(ALT、AST)的变化情况,通过光镜、电镜观察肾组织的病理改变,并运用Real-time PCR法检测肾组织Nephrin mRNA、Podocin mRNA的表达情况。结果:1.芪楼固肾方可改善膜性肾病大鼠的一般状态,减少尿蛋白,升高血清总蛋白和白蛋白,与模型对照组比较差异有统计学意义(P0.05,P0.01);其中芪楼固肾方低剂量组疗效最佳;但对肝功能的影响无明显改变,各组间差异无统计学意义(P0.05);2.芪楼固肾方可在一定程度上减轻膜性肾病大鼠肾脏的病理损伤,减少免疫复合物的沉积,抑制肾小球基底膜增厚;3.芪楼固肾方可上调膜性肾病大鼠肾组织Nephrin mRNA、Podocin mRNA的表达,与模型对照组比较差异有统计学意义(P0.01),以芪楼固肾方低剂量效果为佳;芪楼固肾方高剂量与雷公藤多甙片组比较,差异无统计学意义(P0.05)。结论:1芪楼固肾方能够减少膜性肾病大鼠的蛋白尿,升高血清白蛋白,对膜性肾病大鼠具有一定的治疗作用。2芪楼固肾方能够在一定程度上减轻膜性肾病大鼠肾组织病理的损伤,延缓肾组织病理损伤的进展;3芪楼固肾方可上调膜性肾病大鼠肾组织中Nephrin mRNA、Podocin mRNA的表达,保护足细胞,维持足细胞裂孔膜结构和肾小球滤过屏障功能的完整性,从而减少尿蛋白的排出,延缓膜性肾病的进展。
[Abstract]:Objective: to investigate the effects of Qilou Gushen recipe on urine protein, serum biochemical index, renal pathology and Nephrin mRNA,Podocin mRNA expression in rats with membranous nephropathy (MNS), and to explore the mechanism of QLDSM in treating membranous nephropathy. Methods: fifty male Wistar rats were randomly selected as blank group. The other rats were injected with bovine serum albumin (C-BSA) once every other day after one week of preimmunization to establish the model of membranous nephropathy in rats. After the model was successfully prepared, the remaining 40 rats were randomly divided into 4 groups, 10 rats in each group, namely model group, low dose group, high dose group and Tripterygium wilfordii polyglycoside group. The changes of liver function (ALT,AST) of serum total protein (TP) albumin (ALB), (ALT,AST) in 24 hours urine protein quantitative (UTP), were observed dynamically for 56 days. The pathological changes of renal tissue were observed by light microscope and electron microscope. Real-time PCR method was used to detect the expression of Nephrin mRNA,Podocin mRNA in renal tissue. The result is 1: 1. QLU Gushen Fang improved the general state of membranous nephropathy rats, reduced urinary protein, increased serum total protein and albumin, compared with the model control group, the difference was statistically significant (P0.05, P0.01). Qilou Guishen prescription low dose group is the best, but the effect on liver function has no significant change, there is no significant difference between the groups (P0.05); 2. QLU Gushen decoction alleviates the pathological damage of kidney, reduces the deposition of immune complex, and inhibits the thickening of glomerular basement membrane to a certain extent in rats with membranous nephropathy. QLU Gushen Fang upregulated the expression of Nephrin mRNA,Podocin mRNA in renal tissue of membranous nephropathy rats compared with the model control group (P0.01). There was no significant difference between the high dose of QLU Gushen recipe and the Tripterygium wilfordii polyglycosides group (P0.05). Conclusion: 1 Qilou Gushen recipe can reduce proteinuria and increase serum albumin in membranous nephropathy rats. It has certain therapeutic effect on membranous nephropathy rats. 2 QLU Gu Shen Fang can alleviate the pathological damage of renal tissue to some extent and delay the progress of pathological injury of renal tissue in rats with membranous nephropathy. (3) QLU Gu Shen Fang upregulated the expression of Nephrin mRNA,Podocin mRNA in renal tissue of rats with membranous nephropathy, protected podocytes, and maintained the integrity of membrane structure and glomerular filtration barrier function of podocytes, so as to reduce the excretion of urinary protein. Delay the progression of membranous nephropathy.
