头穴结合夹脊电针法治疗根性坐骨神经痛的临床疗效观察

【摘要】：目的：对比分析头穴结合夹脊电针组与单纯夹脊电针组治疗腰椎间盘突出引起根性坐骨神经痛患者的临床疗效,探寻更有效的根性坐骨神经痛的临床治疗方法。方法：将符合标准60例患者随机分配,治疗组与对照组各30例。治疗组选穴百会、腰夹脊穴、秩边、委中、承山、昆仑、殷门,头穴百会透刺健侧络却穴,要求施术者捻针速率达到每分钟200次,捻转1min,留针2小时；夹脊穴通电,左右连接,采用疏波,电流以患者耐受为度,每次30分钟。对照组选取腰夹脊穴、秩边、委中、承山、昆仑、殷门,夹脊穴通电,采用疏波,左右连接,电流以患者耐受为度,每次30分钟,其他穴位常规针刺,平补平泻。每日两次,治疗六天休息一天。七天作为一个疗程。连续治疗两个疗程后,分别采用简化McGill量表(包括PRI及VAS)积分及直腿抬高试验来观察患者治疗前后腰腿疼痛程度、情感以及神经根压迫的改善状况,评定临床疗效并进行统计分析。结果：1、治疗组与对照组在治疗2周后,PRI、VAS积及直腿抬高试验积分评分较治疗前明显改善。2、与对照组相比,治疗组患者在直观疼痛程度、感觉情感项评分及患肢抬高幅度方面改善更为明显,具有显著性差异。(P0.05)。结论：1、头穴结合夹脊电针与单纯夹脊电针法均能改善腰椎间盘突出引起的根性坐骨神经痛患者的症状。2、头穴结合夹脊电针法治疗腰椎间盘突出引起的根性坐骨神经痛疗效优于传统单纯夹脊电针治疗。
[Abstract]:Objective: to compare and analyze the clinical effect of scalp point combined with chiropractic electroacupuncture group and simple Jiaji electroacupuncture group in the treatment of radical sciatica caused by lumbar disc herniation, and to explore more effective clinical treatment methods for root sciatica. Methods: 60 patients in accordance with the standard were randomly assigned, 30 cases in the treatment group and 30 cases in the control group. After two consecutive courses of treatment, simplified McGill scale (including PRI and VAS) and straight-leg elevation test were used to observe the improvement of pain, emotion and nerve root compression before and after treatment. The clinical effect was evaluated and statistical analysis was carried out. Results: 1. After 2 weeks of treatment, the scores of PRI,VAS product and straight-leg elevation test in the treatment group and the control group were significantly improved. 2. Compared with the control group, the patients in the treatment group were in visual pain degree. The scores of sensory and emotional items and the extent of elevation of the affected limbs were significantly improved (P0.05). Conclusion: 1. The combination of scalp point and chiropractic electroacupuncture can improve the symptoms of patients with radical sciatica caused by lumbar disc herniation. The curative effect of scalp point combined with chiropractic electroacupuncture in the treatment of sciatica caused by lumbar intervertebral disc herniation is superior to that of traditional chiropractor electroacupuncture.
