复方参鹿颗粒治疗肾阳虚型再生障碍性贫血的临床研究

【摘要】：目的:观察复方参鹿颗粒治疗肾阳虚型再生障碍性贫血(AA)的临床疗效和安全性,初步探讨其作用机制。方法:将符合纳入标准的60例患者随机分为治疗组和对照组,每组30例,疗程均为6个月,随访1年以上。观察两组临床疗效、中医证候疗效、外周血象、T淋巴细胞亚群的变化及不良反应。结果:治疗组临床总有效率高于对照组,但差异无统计学意义(P0.05)。治疗组中医证候疗效优于对照组,差异有统计学意义(P0.05)。与治疗前比较,治疗组疗后红细胞、血红蛋白、血小板计数明显升高(P0.01),但与对照组比较差异无统计学意义(P0.05)。与治疗前比较,治疗组疗后CD3~+、CD4~-、CD8~+/CD3~+、CD19-细胞比例较疗前明显降低(P0.05),但与对照组比较差异无统计学意义(P0.05)。治疗组疗后CD4~+、CD25~+、Fox P3~+/CD4~+、CD25~+细胞比例明显上升,(P0.05),且与对照组比较差异有统计学意义(P0.05)。结论:复方参鹿颗粒治疗肾阳虚型AA疗效确切、安全,其作用机制可能与减少CD3~+、CD4~-、CD8~+细胞数量,增加CD4~+、CD25~+调节性T细胞数量、提高其Foxp3的表达有关。
[Abstract]:Objective: to observe the clinical efficacy and safety of compound Shenlu granule in the treatment of (AA) with kidney-yang deficiency type aplastic anemia. Methods: 60 patients who met the inclusion criteria were randomly divided into treatment group (n = 30) and control group (n = 30). The course of treatment was 6 months and the follow-up was more than one year. The clinical efficacy, TCM syndromes, peripheral blood, T lymphocyte subsets and adverse reactions were observed. Results: the total effective rate in the treatment group was higher than that in the control group, but the difference was not statistically significant (P0.05). The therapeutic effect of TCM syndrome in the treatment group was better than that in the control group, and the difference was statistically significant (P0.05). Compared with before treatment, the red blood cell, hemoglobin, platelet count in the treatment group increased significantly (P0.01), but there was no significant difference between the treatment group and the control group (P0.05). Compared with before treatment, the proportion of CD3~, CD4~-,CD8~ / CD3~, CD19- cells in the treatment group was significantly lower than that before treatment (P0.05), but there was no significant difference between the treatment group and the control group (P0.05). The proportion of CD4~, CD25~, Fox P3 ~ / CD4~ and CD25~ cells in the treatment group increased significantly after treatment (P0.05), and the difference was statistically significant compared with the control group (P0.05). Conclusion: compound Shenlu granule is effective and safe in the treatment of AA with deficiency of kidney-yang, and its mechanism may be related to decreasing the number of CD3~ and CD4~-,CD8~ cells, increasing the number of CD4~ and CD25~ regulatory T cells and increasing the expression of Foxp3.
