大柴胡汤治疗胆汁反流性胃炎的临床研究

【摘要】：目的:评价大柴胡汤治疗胆汁反流性胃炎(BRG)的临床疗效,并初步探讨大柴胡汤的作用机理。方法:选择胆汁反流性胃炎患者60例,随机分为治疗组和对照组各30例。治疗组患者予以大柴胡汤口服治疗,对照组患者给以多潘立酮配合铝碳酸镁口服治疗。4周为1个治疗疗程,观察记录两组实验前后的中医证候变化指标等,采用SPSS 17.0程序进行数据统计,继而得出结论。结果:治疗后两组患者临床症状、胃镜胆汁反流程度等较治疗前均有所改善,且治疗组优于对照组,具有统计学意义(P0.01或P0.05)。治疗组中医单项症状积分均较前减低,具有显著的统计学差异(P0.01)。治疗期间未记录到安全指标及不良反应等异常。结论:大柴胡汤治疗胆汁反流性胃炎的临床效果显著,同时安全性高,临床应用安全可靠。其治疗机理可能是:调整胃及十二指肠运动节律,增强胃肠运动水平,促进胃肠道排空,防止胆汁反流入胃,同时保护胃粘膜、促进损伤粘膜修复等。本次临床研究结果验证了大柴胡汤治疗胆汁反流性胃炎的临床治疗效果,且具有进一步研究和探讨的价值。
[Abstract]:Objective: to evaluate the clinical effect of Dachaihu decoction on bile reflux gastritis (BRG) and to explore the mechanism of Dachaihu decoction. Methods: 60 patients with bile reflux gastritis were randomly divided into treatment group (n = 30) and control group (n = 30). The patients in the treatment group were treated with Dachaihu decoction orally, while the patients in the control group were treated by oral administration of domperidone and magnesium bicarbonate. The changes of TCM syndromes before and after the two groups were observed and recorded after 4 weeks as a course of treatment. SPSS 17.0 program is used for data statistics, and then the conclusion is drawn. Results: after treatment, the clinical symptoms and the degree of bile reflux under gastroscopy were improved in both groups, and the treatment group was superior to the control group (P0.01 or P0.05). The scores of TCM single symptom in the treatment group were significantly lower than those in the former group (P0.01). No safety indicators and adverse reactions were recorded during the treatment. Conclusion: Dachaihu decoction is effective and safe in the treatment of bile reflux gastritis. The therapeutic mechanism may be as follows: regulating the motility rhythm of stomach and duodenum, enhancing the level of gastrointestinal motility, promoting gastrointestinal emptying, preventing bile from flowing back into the stomach, protecting gastric mucosa at the same time, promoting the repair of damaged mucosa, etc. The results of this clinical study verify the clinical effect of Dachaihu decoction in treating bile reflux gastritis, and it has the value of further study and discussion.
