温经燥湿汤联合火针治疗寻常性痤疮（寒湿症）的临床观察

【摘要】：目的:观察温经燥湿汤联合火针治疗寻常性痤疮(寒湿证)的临床疗效及相关激素指标血清睾酮(T)、雌二醇(E2)水平的影响,并进行安全性评估。方法:采用随机对照的方法,按要求选入80例寻常性痤疮(寒湿型)女性患者,随机分为治疗组和对照组,因在研究观察过程中脱落病例4例,实际完成病例数为治疗组39例,对照组37例。治疗组给予"温经燥湿汤联合火针"的方法,对照组给予中成药"丹参酮"口服。经过8周的治疗后,通过对两组患者治疗前后皮损情况评分、中医证候评分、皮损疗效三个方面的比较,定性、定量评价温经燥湿汤联合火针疗法的临床疗效。同时测定患者治疗前后血清睾酮(T)、雌二醇(E2)含量,比较两组患者的改善情况。结果:1.治疗8周后,治疗组皮损情况评分的改善情况优于对照组(P0.05),治疗组中医证候评分的改善情况优于对照组(P＜0.05)。其中治疗组患者临床治愈6例、显效22例、有效9例、无效2例,总有效率94.87%;对照组患者临床治愈2例、显效3例、有效24例、无效8例,总有效率78.38%。两组患者疗效比较存在显著性差异(P＜0.05)。2.两组患者血清睾酮(T)、雌二醇(E2)的含量与治疗前相比均有显著性差异(P＜0.05),治疗8周后,治疗组的血清中的相关激素指标缓解情况优于对照组(P＜0.05)。结论:(1)温经燥湿汤联合火针疗法与丹参酮胶囊相比较,其在缓解寻常性痤疮(寒湿证)患者的皮损情况评分、中医证候评分均有较好的疗效,治疗效果显著,且不良反应少,安全性高。(2)温经燥湿汤可调节痤疮患者血清睾酮(T)、雌二醇(E2)的水平,其可能通过调整患者血清激素水平而起到治疗痤疮的作用。
[Abstract]:Objective: to observe the clinical effect of warming meridian dampness decoction combined with fire acupuncture in the treatment of acne vulgaris (cold and dampness syndrome) and the effect of serum testosterone (T), estradiol (E 2) on the related hormone indexes, and to evaluate the safety. Methods: 80 female patients with acne vulgaris (cold and wet type) were randomly divided into treatment group (n = 4) and control group (n = 39). Control group 37 cases. The treatment group was treated with warm meridian dampness decoction combined with fire needle, and the control group with traditional Chinese medicine tanshinone. After 8 weeks of treatment, the clinical efficacy of WJD combined with fire needle therapy was evaluated by comparing the score of skin lesions, TCM syndrome score and curative effect of skin lesions between the two groups before and after treatment. Serum testosterone (T), estradiol (E 2) levels were measured before and after treatment. Results: 1. After 8 weeks treatment, the improvement of skin lesion score in the treatment group was better than that in the control group (P0.05), and the improvement of TCM syndrome score in the treatment group was better than that in the control group (P < 0.05). In the treatment group, 6 cases were clinically cured, 22 cases were effective, 9 cases were effective, 2 cases were ineffective, and the total effective rate was 94.87 cases, while in the control group, 2 cases were clinically cured, 3 cases were markedly effective, 24 cases were effective, 8 cases were ineffective, and the total effective rate was 78.38%. There was significant difference in curative effect between the two groups (P < 0.05). The levels of serum testosterone (T), estradiol (E2) in the two groups were significantly different from those before treatment (P < 0. 05). The remission of serum hormones in the treatment group was better than that in the control group (P < 0.05). Conclusion: (1) compared with Tanshinone capsule, WJD combined with Huoxin capsule has better curative effect in relieving the skin lesion of patients with acne vulgaris (cold and dampness syndrome) and TCM syndrome score, and the therapeutic effect is remarkable. (2) Wenjing Qianshi decoction can regulate the level of testosterone (T), estradiol (E _ 2) in the serum of acne patients, which may play a role in the treatment of acne by adjusting the serum hormone level.
