基于自主神经反应模式研究电针内关、间使对恐惧状态志愿者心率变异性的影响

【摘要】:恐惧是被研究得最为广泛的情绪之一,其失调会诱发许多心理疾病,如创伤后应激障碍、抑郁症和焦虑症等。恐惧的心理应激可以引起多种自主神经反应模式,包括了经典拮抗模式、共同模式和非伴随模式等九种反应模式。心率变异性(Heart Rate Variability,HRV)是指逐次心动周期的细小变异,可定量评估心脏交感神经和副交感神经的活动,广泛用于量化心脏自主神经活动情况。传统医学认为,手厥阴心包经经穴内关、间使主治心胸及心、脉相关疾病。大量临床观察证实,针刺能缓解和控制恐惧,其效应与自主神经功能密切相关。现代实验研究也表明,手厥阴心包经穴位与心脏存在着密切的联系,并且存在形态学基础依据。综上,本研究通过采用Actiheart心电记录仪观察电针内关、间使刺激下恐惧状态志愿者HRV的变化情况,分析电针过程中心脏自主神经的变化,探究针刺缓解恐惧情绪的作用机制,为针刺治疗恐惧情绪引起的相关疾病的提供理论支持。目的:基于自主神经反应模式观察电针内关、间使对恐惧状态志愿者HRV的影响,分析电针内关、间使的作用机制。方法:招募200名志愿者,男女比例1:1。80名进行音频试验,120名进行电针试验。音频试验中,80名志愿者随机均分为中性音频组和恐怖音频组,分别接受中性和恐怖音频刺激。音频试验成功后进行电针试验,120名志愿者随机均分为电针组和对照组,两组志愿者接受恐怖音频刺激后,电针组接受电针内关、间使刺激,对照组不接受干预。观察两组在电针前、中、后心率变异性的变化。结果:根据恐怖音频刺激前后VLF和HF的差值情况把志愿者分类成对抗模式Ⅰ(交感兴奋,副交感抑制)、对抗模式Ⅱ(交感抑制,副交感兴奋)、共同兴奋模式(交感副交感共同兴奋)和共同抑制模式(交感副交感共同抑制)。对抗模式Ⅰ:电针组与对照组比较,针刺中SD、RMSSD和VLF显著提高;HF在针刺中及针刺后均显著提高,余指标无显著性差异(p0.05)。对抗模式Ⅱ:各项指标在针刺前、中、后均无显著性差异(p0.05)。共同兴奋模式:电针组与对照组比较,针刺中LF显著提高;针刺后各项指标无显著性差异(p0.05)。共同抑制模式:电针组与对照组比较,针刺中、针刺后HF均显著提高,余指标在针刺前、中、后均无显著性差异(p0.05)。结论:恐惧情绪下,电针内关、间使对不同自主神经反应模式有不同的影响。
[Abstract]:Fear is one of the most widely studied emotions, which can induce many mental disorders, such as post-traumatic stress disorder, depression and anxiety disorder. Fear of psychological stress can lead to a variety of autonomic nervous response patterns, including the classic antagonistic model, common model and non-adjoint model. Heart rate variability (Heart Rate Variability,HRV) refers to the small variation of cardiac cycle, which can quantitatively evaluate the activity of cardiac sympathetic and parasympathetic nerves, and is widely used to quantify cardiac autonomic nerve activity. According to traditional medicine, hand-jueyin pericardium meridian internal clearance, the main treatment of heart-chest and heart, pulse-related diseases. A large number of clinical observations confirmed that acupuncture can relieve and control fear, and its effect is closely related to autonomic nervous function. Modern experimental studies also show that there is a close relationship between the acupoints of the pericardial meridian of the hand and the heart, and there is a morphological basis. To sum up, this study observed the changes of HRV of volunteers in fear state under electroacupuncture (EA) Neiguan by using Actiheart ECG recorder, analyzed the changes of cardiac autonomic nerves during electroacupuncture, and explored the mechanism of acupuncture to relieve fear emotion. To provide theoretical support for acupuncture treatment of related diseases caused by fear emotion. Aim: to observe the effect of electroacupuncture (EA) on the HRV of volunteers in fear state based on autonomic nerve response model and to analyze the mechanism of EA Neiguan. Methods: 200 volunteers were recruited, the ratio of male and female was 1: 1.80 for audio frequency test and 120 for electroacupuncture. In the audio test, 80 volunteers were randomly divided into neutral audio group and terror audio group. 120 volunteers were randomly divided into electroacupuncture group and control group. After the two groups received scary audio stimulation, the electroacupuncture group received electroacupuncture Neiguan stimulation, the control group did not accept intervention. The changes of heart rate variability before, during and after electroacupuncture were observed.
