稳定型心绞痛循证病机—证治体系构建研究

【摘要】：目的:运用系统评价方法,构建稳定型心绞痛循证病机-证治体系,为提高临床疗效提供科学依据。方法:(1)检索CBM、CNKI、VIP、Wanfang data各国内中文数据库中中医药治疗稳定型心绞痛的临床研究,筛选合格研究,采用Meta-分析的方法进行有效性及安全性的系统评价。(2)以有效、安全及统计结果稳定为标准,筛选中医药治疗稳定型心绞痛的优势治法、方药。结果:共检索到相关临床对照试验研究28704篇,最终纳入合格研究402篇。Jadad评分,60项研究得分≥3分,其余研究得分均3分。涉及汤剂、丸剂、散剂、注射剂等方药155种,体现出益气养阴、活血祛瘀、益气活血祛瘀等12种治法及其相应治则的临床指导作用。结论:(1)虚性病机—证治体系为气虚-气虚证-补气法-黄芪注射液;气阴两虚-气阴两虚证-益气养阴法-参麦注射液。(2)实性病机—证治体系为心血瘀阻-心血瘀阻证-活血祛瘀法-葛根素注射液、银丹心脑通胶囊、丹参川芎嗪注射液、血府逐瘀汤、杏丁注射液、心可舒片、疏血通注射液、红花黄色素注射液、复方丹参滴丸、血塞通胶囊。(3)虚实夹杂病机—证治体系为气阴两虚,瘀血阻滞-气阴两虚,瘀血阻滞证-益气养阴,活血祛瘀法-稳心颗粒;气虚痰瘀阻络-气虚痰瘀阻络证-芳香开窍,益气活血祛瘀法-麝香保心丸;气虚血瘀-气虚血瘀证-益气活血祛瘀法-补阳还五汤、通心络胶囊。
[Abstract]:Objective: to establish a system of Evidence-Based Pathogenesis and treatment of stable angina Pectoris by systematic evaluation, and to provide scientific basis for improving clinical efficacy. Methods: (1) the clinical study on the treatment of stable angina pectoris with traditional Chinese medicine (TCM) in CBM,CNKI,VIP,Wanfang data Chinese database was searched, and qualified research was screened.
