交通心肾法治疗消渴并淋证

【摘要】：糖尿病合并尿路感染是由于长期的糖代谢紊乱,导致人体的防御功能减弱,或继发神经源性膀胱致尿潴留,使各种病原微生物侵犯尿路粘膜或组织引起的炎性疾病,通常伴有菌尿和脓尿。糖尿病合并尿路感染现已成为糖尿病并发感染性疾病中发病率第二的感染性疾病,仅次于呼吸道感染,严重影响了人们的生活质量和寿命。吾师高天舒教授,从事临床工作及科研教学二十余年,在糖尿病并发症的治疗上积累了丰富的经验,对糖尿病合并尿路感染有着独到的见解。高教授认为,糖尿病合并尿路感染属于祖国医学的“消渴并淋证”范畴,是临床以尿频、尿急、尿痛、尿道灼热、心烦、腰痛、少腹胀痛等为主症的一种疾病。高天舒教授认为本病多因饮食、起居等摄生防护不当,肾阴亏虚,心火亢盛,下移小肠,膀胱气化失常,进而导致淋证。故其发生与心、肾、小肠、膀胱关系密切,是本病的复发根源,并指出临床治疗上当以交通心肾之法去心火以清小肠之热,进而清除膀胱之热,从而消除复发之虑,而非独从肾、膀胱论治。急性期肾水亏虚不能上济心火,心火亢盛下移小肠,治疗当以泻心火为主,引火归元,用八正散合黄连阿胶汤加减;缓解期肾阴亏虚,湿热留恋,治疗当以滋肾水为主,防止心火亢盛,佐以清热,用六味地黄汤合导赤散加减。
[Abstract]:Diabetes mellitus complicated with urinary tract infection is a chronic disorder of glucose metabolism, which results in the weakening of human defense function, or the secondary neurogenic bladder causing urinary retention, which causes various pathogenic microorganisms to invade the mucous membrane or tissue of the urinary tract and cause inflammatory diseases. Usually accompanied by bacteriuria and pyuria. Diabetes mellitus complicated with urinary tract infection has become the second most infectious disease in diabetes complicated with infectious diseases, second only to respiratory tract infection, which has seriously affected the quality of life and life span of people. Professor Gao Tianshu, my teacher, engaged in clinical work and scientific research teaching for more than 20 years, accumulated rich experience in the treatment of diabetic complications, and had unique views on diabetes mellitus complicated with urinary tract infection. Professor Gao believes that diabetes mellitus complicated with urinary tract infection belongs to the category of "dispelling thirst and drenching syndrome" in traditional Chinese medicine. It is a kind of disease with frequency of urination, urgency of urination, urinal pain, burning of urethra, upset, low back pain, less abdominal distending pain and so on. Professor Gao Tianshu believes that the disease is caused by improper protection of diet and daily necessities, deficiency of kidney yin, hyperactivity of heart and fire, downward movement of small intestine and abnormal bladder gasification, which leads to lymphoid syndrome. Therefore, its occurrence is closely related to heart, kidney, small intestine and bladder, which is the root of recurrence of the disease. It is pointed out that the method of communicating heart and kidney should be used to clear the heat of the small intestine in order to clear away the fever of the bladder, so as to eliminate the worry of recurrence. Not only from the kidney, bladder treatment. Acute deficiency of kidney water can not go up the heart fire, the heart fire hyperactivity moved down small intestine, treatment should be mainly to reduce the heart fire, fire return to yuan, with Bazheng Powder Huanglian Ejiao decoction to reduce; In remission stage, deficiency of kidney yin, retention of dampness and heat, treatment should focus on nourishing kidney water, preventing hyperactivity of heart fire, with heat clearing, adding and subtracting with Liuwei Dihuang decoction combined with Daochi San.
