水蛭治疗慢性肾脏病54例对比研究

【摘要】：目的:根据傅汝林教授运用水蛭治疗疑难杂症的经验,我们采用对比研究方法,评价水蛭配合西药治疗慢性肾脏病的临床疗效与安全性。方法:将54例符合CKD CKD3~4期诊断标准的患者用随机数字表法随机分为治疗组和对照组各27例,两组患者均采用慢性肾脏病基础治疗。治疗组:在基础治疗上加服水蛭粉。对照组:仅予基础治疗。共观察4周,比较中医证候积分,血肌酐、24h尿蛋白定量、血常规、肝功能、凝血功能、大便常规+潜血等指标的变化。结果:治疗组与对照组在年龄、性别、原发病及相关指标方面无统计学差异。治疗组脱落1例,对照组自行退出1例。中医证候疗效比较,两组治疗前无统计学差异,治疗后治疗组总有效率为73.1%,对照组总有效率为26.9%,两组治疗后疗效比较有显著性差异(P0.01)。两组治疗前血肌酐比较无统计学差异,治疗后治疗组平均血肌酐水平较对照组平均血肌酐水平下降,经比较有统计学意义(P0.05)。两组治疗前、后24h尿蛋白定量比较无统计学差异。两组患者治疗后血常规、肝功能、凝血功能、大便常规+潜血检查均未见异常。结论:水蛭粉对慢性肾脏病可明显改善临床症状及肾功能,大量(9g/d)吞服疗效确切,且无明显毒副反应。
[Abstract]:Aim: to evaluate the clinical efficacy and safety of Hirudo combined with western medicine in the treatment of chronic kidney disease according to Professor Fu Rulin's experience in the treatment of difficult and complicated diseases. Methods: 54 patients with CKD CKD3~4 stage were randomly divided into two groups: treatment group (n = 27) and control group (n = 27). Treatment group: add hirudin powder to basic treatment. Control group: basic treatment only. The changes of TCM syndrome score, serum creatinine, 24-hour urine protein quantity, blood routine, liver function, coagulation function and stool routine occult blood were observed for 4 weeks. Results: there was no significant difference in age, sex, primary disease and related indexes between the treatment group and the control group. There were 1 case in treatment group and 1 case in control group. After treatment, the total effective rate was 73.1% in the treatment group and 26.9% in the control group. There was significant difference in the curative effect between the two groups after treatment (P0.01). There was no significant difference in serum creatinine between the two groups before treatment. After treatment, the mean serum creatinine level in the treatment group was lower than that in the control group, and there was statistically significant difference between the two groups (P0.05). There was no statistical difference between the two groups before treatment and 24 hours after treatment. After treatment, blood routine, liver function, coagulation function and stool routine occult blood examination were not abnormal in both groups. Conclusion: hirudin powder can significantly improve the clinical symptoms and renal function in patients with chronic renal disease. A large number of (9g/d) swallowing is effective and has no obvious toxic and side effects.
