中药培土生金方治疗老年慢性阻塞性肺疾病稳定期合并营养不良的临床研究

【摘要】：目的:观察中药培土生金方治疗老年慢性阻塞性肺疾病(COPD)稳定期合并营养不良的临床疗效,并探究其作用机理。方法:选择62例老年COPD稳定期合并营养不良患者,随机分为对照组31例,治疗组31例。对照组应用复方营养混悬剂,治疗组给予复方营养混悬剂联合培土生金方。以4周为一疗程,治疗3个疗程后,观察两组患者呼吸功能、营养学指标及临床症状的变化。结果:治疗组总有效率为86.67%,对照组总有效率为66.67%,治疗组患者的血清白蛋白(ALB)、血清总蛋白(STP)、总淋巴细胞计数(LC)、体重指数(BMI)、上臂肌围(AMC)和肱三头肌皮褶厚度(TSF)较治疗前均明显改善,具有统计学意义(P0.01或P0.05);除STP外,组间比较,疗效优于对照组,有统计学意义(P0.05或P0.01)。治疗组患者CAT评分及中医证候积分较治疗前明显降低,组内比较有极显著性差异(P0.01),且疗效优于对照组(P0.05或P0.01);治疗组患者6分钟步行距离(6MWD)明显增加,组内比较有极显著性差异(P0.01),且疗效优于对照组(P0.01)。治疗组患者FEV1、FEV1/FVC组内比较,有极显著差异性(P0.01),疗效优于对照组(P0.05)。结论:中药培土生金方联合复方营养混悬剂可明显改善老年慢性阻塞性肺疾病稳定期合并营养不良患者的营养状况,提高免疫力,改善呼吸功能,增强活动耐力,降低疾病的严重程度,使病情得以控制,生活质量得以改善。
[Abstract]:Aim: to observe the clinical efficacy and mechanism of Peitushengjin recipe in the treatment of senile chronic obstructive pulmonary disease (COPD) with malnutrition. Methods: 62 elderly COPD patients with malnutrition were randomly divided into control group (n = 31) and treatment group (n = 31). The control group was treated with compound nutrition suspension, and the treatment group was treated with compound nutrition suspension combined with Peitusheng Jin Fang. After 3 courses of treatment, the respiratory function, nutritional indexes and clinical symptoms of the two groups were observed. Results: the total effective rate was 86.67% in the treatment group and 66.67% in the control group. The (LC), body mass index (LC),) (BMI), was counted in the serum albumin (ALB), serum total protein (STP), total lymphocyte count in the treatment group. The upper arm muscle circumference (AMC) and triceps brachii skinfold thickness (TSF) were significantly improved compared with those before treatment (P0.01 or P0.05). With the exception of STP, the curative effect was better than that of the control group (P0.05 or P0.01). The CAT score and TCM syndrome score in the treatment group were significantly lower than those before treatment (P0.01), and the curative effect was better than that in the control group (P0.05 or P0.01). The 6-minute walking distance (6MWD) was significantly increased in the treatment group (P0.01), and the curative effect was better than that in the control group (P0.01). There was significant difference in FEV1,FEV1/FVC between the treatment group and the control group (P0.01), and the curative effect was better than that of the control group (P0.05). Conclusion: Peitushengjin prescription combined with compound nutritional suspension can significantly improve the nutritional status of elderly patients with chronic obstructive pulmonary disease complicated with malnutrition, improve immunity, improve respiratory function, and enhance activity endurance. Reduce the severity of the disease, so that the disease can be controlled, quality of life can be improved.
