深圳市世界银行贷款结核病控制绩效评价

【摘要】:目的 对深圳市为期 8年的全人口世行贷款结核病控制项目工作进行绩效评价 ,探讨项目绩效评价的方法。方法 收集 1993~ 2 0 0 1年深圳市全人口结核病控制项目周期内转诊、可疑者接诊、涂阳患病、痰菌阴转、治愈情况等及费用筹集和使用的有关资料 ,并在项目实施前后进行结核病流行病学调查和耐药监测 ,从服务公平性、健康可及性、项目效果、项目效益、项目效率等方面对该项目进行评价。结果 (1)服务公平 :将外来人口纳入结核病控制项目管理 ,实施全人口控制策略 ,并取得了与户籍人口一致的效果 ;(2 )健康可及 :健全了市、区、镇三级防治网络 ,全面实施归口管治 ,推行全程督导管理方式 (DOTS)策略 ;(3)效果明显 :减少了 1 5万新发病人 ,患病率下降超过 5 0 % ,初始涂阳耐多药率下降 7% ;(4 )效益巨大 :直接经济效益 1 4 2亿元 ,间接经济效益挽回GDP损失 30 6亿元 ,减少个人收入 17 6亿元 ;(5 )效率高 :2月末痰菌阴转率 88 8% ,3月末痰菌阴转率 96 0 %。结论 该项目实施 8年绩效明显 ,政府承诺保证该项目可持续发展是非常必要的。
[Abstract]:Objective to evaluate the performance of the whole population World Bank loan TB Control Project in Shenzhen City for 8 years and to explore the method of project performance evaluation. Methods data were collected from the whole population TB control project in Shenzhen from 1993 to 2001, such as referrals, suspicious cases, smear positive disease, sputum negative conversion, cure status, and cost collection and use. The tuberculosis epidemiological investigation and drug resistance monitoring were carried out before and after the implementation of the project to evaluate the project from the aspects of service fairness, health accessibility, project effect, project benefit and project efficiency. Results (1) Fairness of service: bring foreign population into TB control project management, implement whole population control strategy, and achieve the same effect as household registration population; (2) Health accessibility: improved city and district, The three-level prevention and treatment network of town, the overall implementation of centralized governance, the implementation of the whole process of supervision and management of the (DOTS) strategy; (3) the effect is obvious: 150000 new patients were reduced, the prevalence rate decreased by more than 50%, The rate of initial smear positive multidrug resistance decreased by 7%. (4) the benefit is huge: the direct economic benefit is 1 4.2 billion yuan, the indirect economic benefit is 30.6 billion yuan, the loss of GDP is 30.6 billion yuan. The rate of sputum negative conversion was 88 8% at the end of February and 96 0% at the end of March. Conclusion the performance of the project is obvious in 8 years, and it is necessary for the government to guarantee the sustainable development of the project.
